1. The subject of the contract is the provision of cleaning services:

1.1. Part I – refers to the provision of services in the facilities of the National Health Fund in Wrocław at 6 Joannitów Street (building A and B) and at 2-6 Dawida Street (building C), as well as in the City Forum 2 building at 55 Traugutta Street.

1.2. Part II – refers to the provision of services in the facility of the Branch Office of the National Health Fund in Jelenia Góra at 18 Wolności Street,

1.3. Part III – refers to the provision of services in the facility of the Branch Office of the National Health Fund in Legnica at 7 J. Pawła II Street and in the premises of the Customer Service Point in Głogów at 15 Kotlarska Street,

1.4. Part IV – refers to the provision of services in the facility of the Branch Office of the National Health Fund in Wałbrzych at 1-3 Kościuszki Street, and in the premises of the Customer Service Point in Kłodzko at 11 Objazdowa Street, room 2.
