2. Detailed description of the item of the order include:

Appendix No. 1a – Assortment and Price Form – for Tasks 1, 2, 3, 4.

Appendix No. 1b - Description of technical parameters of the subject of the contract - for Tasks 1, 2, 3, 4.

3. Detailed terms and conditions concerning the performance of the subject matter of the contract are regulated in the Model Agreement, Appendix No. 4 to the Terms of Reference "Model Agreement".

Task 1- Gastroscope

1. The subject of the order is EQUIPMENT FOR ENDOSCOPY LABORATORY – Gastroscope HDTV-3 pcs.

2. Detailed description of the item of the order include:

Appendix No. 1a – Assortment and Price Form – for Tasks 1, 2, 3, 4.

Appendix No. 1b - Description of technical parameters of the subject of the contract - for Tasks 1, 2, 3, 4.

3. Detailed terms and conditions concerning the performance of the subject matter of the contract are regulated in the Model Agreement, Appendix No. 4 to the Terms of Reference "Model Agreement".

Task 2- Colonoscope

1. The subject of the order is EQUIPMENT FOR ENDOSCOPY LABORATORY – Colonoscope – 3 pcs.

2. Detailed description of the item of the order include:

Appendix No. 1a – Assortment and Price Form – for Tasks 1, 2, 3, 4.

Appendix No. 1b - Description of technical parameters of the subject of the contract - for Tasks 1, 2, 3, 4.
