1. The subject of the order is the delivery, assembly and installation of a cryochamber for the needs of the SP ZOZ City Clinic in Pieszyce described in detail in Annex 1 to the ToR. "Detailed description of the subject of the contract".

2. Subject of the order according to the Common Procurement Vocabulary: CPV: 33.10.00.00-1 medical devices.

3. The subject of the contract includes:

a. delivery, installation and commissioning of the Device;

b. training of persons operating the Device in its use, ongoing operation and maintenance;

c. granting a guarantee for a minimum period of 24 months and on the terms set out in Annex 5 to the ToR.

1. The subject of the order is the delivery, assembly and installation of a cryochamber for the needs of the SP ZOZ City Clinic in Pieszyce.

The Contractor undertakes at his own expense and at his own risk to deliver the subject of the contract to the Principal to its headquarters on the designated day, from 8:00 to 15:00.

The subject of the contract includes:

a) Delivery, assembly and commissioning of the Device;

b) Training of persons operating the Device in its use, ongoing operation and maintenance;

c) Provision of guarantees for a minimum period of 24 months and the principles set out in Annex 5 to the ToR.

Adaptation works of the room in which the Device will work will be carried out by the Employer. The Contractor's task will be the assembly and installation of the Equipment in the outpatient room indicated by the Employer and obtaining all necessary, i.e. legally required, acceptance documentation.

2. Additional requirements regarding the implementation of the subject of the contract.

a) The Employer requires that the device offered was brand new, models manufactured in 2018;

b) The Ordering Party requires that the Device be issued in the original manufacturer's packaging;

c) The Ordering Party requires that the offered Device has the CE mark; meets all PN standards, has the required registration certificate;

d) The Contractor shall provide the Employer together with the subject of the contract:

- Warranty Card,

- Device manual in Polish,

- List of authorized service centers in Poland,

- Rules for the provision of services by an authorized service post-warranty period,

- Catalog (commercial) equipment specificity,

- List of consumables used in the current operation of the Equipment,

- Other necessary documents (opinions, certificates, approvals - in accordance with the requirements set out in the Act on Medical Devices),

- Reports of examinations and measurements carried out specified in regulations,

- Service documentation.
