The subject of the contract is the provision of cleaning and cleaning services to the Silesian Museum in Katowice:

1) inside the objects belonging to the Museum in the following locations:

• in the main seat of the Museum at Tadeusza Dobrowolskiego 1, 40-205 Katowice,

• in the building belonging to the Museum at al. Wojciech Korfantego 3, 40-005 Katowice;

2) the area of land adjacent to the facilities in the above-mentioned locations.

1. The subject of the contract is the provision of cleaning and cleaning services to the Silesian Museum in Katowice:

1) inside the objects belonging to the Museum in the following locations:

• in the main seat of the Museum at Tadeusza Dobrowolskiego 1, 40-205 Katowice,

• in the building belonging to the Museum at al. Wojciech Korfantego 3, 40-005 Katowice;

2) the area of land adjacent to the facilities in the above-mentioned locations.

The contractor will be obliged to perform:

a) basic cleaning services (main service) carried out successively with a frequency (for individual activities) not less than that indicated in the description of the subject of the contract (Connector No. 1 to SWZ), in the period in accordance with the contract – for which the Contractor will receive a monthly lump sum remuneration (items 1-5 of the Offer Price Spreadsheet). The scope of the main service includes ongoing maintenance of cleanliness inside the Museum's buildings located in the above-mentioned locations together with the adjacent external area, ongoing snow removal of the area and cleaning of selected glass surfaces,

b) additional services provided only at the request of the Ordering Party, for which the Contractor will receive remuneration on the basis of unit prices indicated in the offer. Additional services include, in particular, mountaineering washing of glass surfaces, ad hoc cleaning services, as well as snow removal of roofs and snow removal and disposal.

2. A detailed description of the subject of the contract ("OPZ") is included in Appendix No. 1 to the SWZ. In addition, the Contractor is obliged to perform the subject of the contract, in accordance with the relevant provisions of the contract - Appendix No. 2 to the SWZ.

3. Deadline for the execution of the order, renewal of the order:

1) The Contractor is obliged to perform the contract: from the date of conclusion of the contract, but not earlier than from 01.01.2023 to 31.12.2023, hereinafter referred to as the "basic order";

2) The Ordering Party reserves the right to resume this contract, i.e. the Contractor's obligation to repeat the subject of the contract once between 01.01.2024 and 31.12.2024;

3) the subject of the contract as part of the renewal is identical to the subject of the contract performed within the period indicated in Chapter III paragraph 4 point 1) of the SWZ, i.e. it will consist in performing the same works and activities as in the basic order to the extent indicated in the OPZ and in the offer form;

4) The Contractor will be obliged to resume the performance of the contract only after the Ordering Party has provided a statement in this respect, by 15.12.2023 at the latest. The statement will be submitted in writing in accordance with the rules of representation of the Ordering Party;

5) if the Ordering Party fails to submit a statement on renewal in accordance with the rules set out in Chapter III,4)(4) of the SWZ, the Contractor shall not be obliged to perform the renewal of the contract;

6) The Ordering Party reserves that the Contractor will not be entitled to remuneration in the event of resignation by the Ordering Party from the resumption of the contract, as well as no compensation in this respect.

4. Pursuant to Article 95(1) of the PPL Act, the Contracting Authority shall require the Contractor or subcontractor to employ persons performing the indicated activities in the scope of the performance of the contract, hereinafter referred to as "Employees" on the basis of the employment relationship. The type of activities covered by the requirements for employing persons on the basis of an employment relationship, the method of verifying the employment of persons, the Ordering Party's rights to control the Contractor's compliance with the requirements related to the employment of persons and sanctions for non-compliance with these requirements - are regulated by the provisions of the essential provisions of the contract - Annex. 2 to SWZ.
