The subject of the contract is the provision of successive collection services from the landfill site of Przedsiębiorstwo Usług Komunalnych Sp. z o. o. in Tarnów, bulky waste with the code 20 03 07, specified in the provisions of the Regulation of the Minister of Climate of 2.1.2020 on the waste catalog (Journal of Laws of 2020 item 10) and their management in accordance with the requirements of the Act of on waste, December 14, 2012 (Journal of Laws of 2019, item 701, as amended) in the first half of 2020.

1. The minimum amount of waste (minimum scope of the contract), i.e. the amount of waste that the Contractor is obliged to collect from the Employer under the contract is 400 (Mg).

2. The maximum amount of waste (maximum possible scope of the contract), i.e. the amount of waste that the Contractor is obliged to collect from the Employer as part of the contract if the Employer exercises the option right, in the situation described in point 3.3, is up to 1,500 (Mg) .

3. The following conditions are set for the Contractor to provide services covered by the subject of the contract:

1) Collection of waste will be carried out through their removal from the Employer's landfill located at ul. Komunalna 31, 33-100 Tarnów (place of waste collection) at the Contractor's cost and risk, with the Contractor's own transport, i.e. vehicles adapted for waste transport with the code 20 03 07, equipped with two containers KP-36 or a set with moving floor walkingfloor.

2) Collection of waste by the Contractor from the place of collection will take place on business days of the Employer, i.e. from Monday to Friday from 7 to 14, except on public holidays, on the basis of the request referred to in point 3.

3) Each order to collect waste from the place of collection will be carried out by the Ordering Party in a request sent by e-mail to the Contractor's e-mail address or notified by phone to the Contractor's telephone number with simultaneous confirmation of the request via e-mail to the Contractor's e-mail address. In the call, the Employer shall specify in each case the amount (Mg) of waste to be collected by the Contractor, while the one-time amount of waste [Mg] to be collected on a single call from the place of waste collection shall not exceed 35 (Mg).

4) Collection of waste by the Contractor from the place of receipt in the quantity (Mg) indicated in the call referred to in point 3 will take place within 3 working days from the day of sending the Contractor by the Employer of the given call.

5) Loading the waste at the place of its receipt onto the Contractor's vehicles lies with the Employer.

6) The confirmation of receipt of waste will be implemented via an electronic database of products and packaging and waste management (hereinafter BDO). In the event of a failure of the BDO ICT system (with the exception of the failure occurring on the part of the Employer or the Contractor as BDO users) preventing the generation of a transfer card waste (hereinafter KPO), KPO will be prepared in paper or electronic form kept outside BDO.
