The subject of the contract is construction works involving the implementation of the investment: 'Modernization of the water supply and sewage system - Stage III, ul. May 1 in Strzelce Opolskie (contract 10 and 4) - divided into:

a) Construction of sanitary sewage system, reconstruction of the main and distribution water supply network in ul. May 1 in Strzelce Opolskie from the MaW56 node to the MaW63 node for the main network and from the MrW1 node to the MrW19 node for the distribution network - (contracts 4 and 10)

b) Reconstruction of water supply connections on private properties.

The scope of the order:

"Construction of sanitary sewage system, reconstruction of the main and distribution water supply network at 1 Maja street in Strzelce Opolskie from node MaW56 to node MaW63 for main network and from node MrW1 to node MrW19 for distribution network - (contract 4 and 10) and reconstruction of water supply connections on the premises private "including:

a. the scope of the gravitational sanitary sewage network from the MaK2.2 well to the MaK6 well, the pressure sewage network from the pumping station to MaT11 and the sewage pumping station with the pumping station area:

- gravity sewer pipeline ø 200 PVC-U SN8 solid - approx. 209.12 m,

- discharge sanitary sewage network ø 90 PE HD100 RC SDR11 (excavation) - approx. 183.35mb,

- construction of concrete wells ø 1200 - 8 pcs

- execution of sewage branches ø 160 - approx. 79.23 m (11 pcs)

- construction of sewage pumping station - 1 set

b. The scope of the main water supply network from the MaW56 node to the MaW 63 node:

- water main ø 355PEHD100RC SDR11 - core drilling - approx. 212.58 mb

- water main ø 355PEHD100RC SDR11 - excavation - approx. 14,21mb.

c. the scope of the distribution water supply network from the MrW1 node to the MrW19 node:

- water supply network ø 160 PEHD 100SDR11 - excavation approx. 4.33mb,

- water supply network ø 110 PEHD 100SDR11 - excavation approx. 292.28 mb,

- water supply network ø 100 mm ductile iron - about 8.94 mb,

- side branches (from the main network to the property borders) ø 32 PEHD SDR11 - pcs. 12 approx. 98 m

- Fire hydrants DN 80 aboveground in quantity of 2 pcs

d. scope of connections on private premises:

- connections ø32 PE 100 SDR 11 - total length approx. 165.30 linear meters

Before connecting the connection to the new network, the Contractor is to verify that the connected installation has a non-return check valve, and in the absence of it, install the above valve. The water connection should be made of PE 100 SDR 11 pipes with an external diameter of 32 mm, connected by welding, from the valve in front of the property owner's plot to the internal water supply system (behind the first wall of the building), i.e. to the water meter console (including the installation of a new console) with adjustable connection in the range of e.g. 130-150 mm enabling the installation of a water meter with 130mm spacing, mounted to the wall with mounting pins at least 60 mm long Before and after the water meter (provided by the Employer), shut-off valves should be installed in succession, and behind the valve from the internal installation side a non-return check valve. Water meters have a nominal diameter of DN 20 and a thread of a 1 "diameter connector. Trench pipelines should be laid on 10 cm sand bed and 30 cm backfill. The connections should be placed at a depth of minimum 1.40 m after the ground level. Contractor is to carry out work in such a way that interruptions in the supply of water to recipients are not longer than 2 hours. In the event of the necessity of longer interruptions in the supply of water, the Employer must be notified. water after installation of the water meter by the Employer's services. After completion of construction works, the construction site should be cleaned up and restored to its original condition.

For the performance of the subject of the contract, the Contractor shall receive a flat-rate remuneration including

all costs related to the implementation of the contract, resulting from the project documentation, STWiOR, SIWZ,

bill of quantities as well as those not resulting from these documents, which the Contractor will incur when carrying out

orders, in particular costs:

a. Full geodetic service of completed construction works, including an inventory

surveying of completed works and obtaining confirmation of its admission to the State Resources

surveying,

b. Occupation of the road lane for the duration of the works,

c. Road restoration in accordance with the decisions of Road Managers and detailed designs and conditions

contractual,

d. Traffic organization and necessary detours for the duration of the works and development of a temporary design

traffic organization or eventual change or update of temporary traffic organization projects

for the duration of the works,

e. Obtaining necessary decisions and reconciliations or changing out-of-date decisions and reconciliations

to perform the intended construction works, which the Contractor will obtain on behalf of the Employer,

f. Preparation of as-built documentation of completed works (for each acceptance of works) containing

particular:

• design documentation with any changes,

• a statement by the Site Manager regarding the completion of works and their completion in accordance with the design documentation and

building permit and regulations,

• a statement by the Site Manager on bringing the building site to a proper condition and order, as well as in

when using, road, street, neighboring property, building or premises,

• geodetic inventory of performed works (geodetic sketches and tabular listing of individual

episodes)

• Surveyor information regarding the location of the construction object pursuant to art. 57 section 1 point 5 of the Act

Building Law that the location of the completed building object is in accordance with the approved design

plot or area development. The information should be signed by a surveyor with a statement,

that he has the appropriate professional qualifications authorizing him to perform an independent function in

field of geodesy and cartography.

• as-built map sealed by the State Geodetic Resource (Poviat Eldership in Strzelce

Opole)

• documents of installed equipment together with operating instructions,

• construction log (a copy of the construction log confirmed as being true to the original upon receipt

partial)

• results of control measurements, tests and examinations in accordance with technical and binding specifications

standards and regulations together with the assessment of the value after conversion by an authorized person, or the result obtained

tests meet the requirements set out in the contractual conditions (excavation compaction of excavation backfill,

testing foundation compaction, laboratory test report on samples cut from the pavement, testing

network tightness, testing of water quality after disinfection of the water supply system).

• recipes and technological arrangements,

• reports on the acceptance of disappearing and coverable works (thickness of ballast and sand backfilling,

thickness of construction layers of plays, correct installation of water supply fittings,

• documentation of embedded materials, justifying their embedding in accordance with the binding

provisions together with a tabular summary,

• technological opinion prepared on the basis of all test and measurement results,

• photographic documentation related to ongoing works (may be in electronic form on the attached)

data carrier),

• documentation of the inspection of the sewage network carried out twice using a camera

video

- the first backfilling and compacting backfilling over the pipes;

- second after laying the reconstruction wear layer

with the recording of the inspection on an electronic medium;

• reports on the receipt and transfer of works accompanying the owners of the infrastructure of the area - relocation of the lines

telephone network

gas network, lighting, etc.,

• confirmation in the form of declarations of all property owners adjacent to the investment site and

areas taken up by the Contractor for the storage of materials or construction facilities on bringing to

the original condition of the areas occupied during the works.

• submitting a card of the transferred waste generated during the implementation of the investment

g. All types of work security, in particular security in terms of safety

road and pedestrian traffic, health and safety and theft and devastation,

h. Waste management arising during the implementation of works (the Contractor is a waste producer in

the meaning of the Waste Act of December 14, 2012 as amended d.,)

i. Archaeological supervision and conservator of monuments,

j. Supervision and participation in the acceptance of representatives of the owners of the infrastructure of the area of energy networks, networks

gas, telephone lines etc.

k. Works and civil liability insurance,

l. Repair any damage to existing infrastructure in the area caused during the execution of works.

Also :

1. The Contractor at the request of the Employer (in the event of such needs) will present

reports on the implementation of the subject of the contract. Each time the form, scope and date of execution of these reports will be

subject of agreement between the parties to the contract.

2. The Contractor shall provide the Purchaser with settlement documents in 2 copies in paper form

and in an electronic version.

3. The Contractor's staff must have a thorough command of Polish or the Contractor shall provide an interpreter / s

to ensure proper communication between the parties to the contract.

4. The Contractor is obliged to cooperate with the Employer during the procedure of obtaining a permit

use or procedures to notify the competent authorities when construction is complete.

5. Contractor - the Contractor's representative (Construction Manager, Construction Engineer) is obliged to

participate in monthly consultations (Construction Councils) regarding the progress of works during which

will present quantitative progress each time since the last meeting and since the beginning of the implementation.

The Purchaser reserves the right to organize Construction Boards on a more frequent basis in the case of

circumstances justifying such a need.

6. The Contractor within 14 days of payment by the Employer of remuneration resulting from the settlement

final order / contract shall provide the Employer with a written statement that he has received all

financial claims under or in connection with the contract.

7. In the case of co-financing a task from the Cohesion Fund under the Program

The Contractor will apply the Operational Infrastructure and Environment in correspondence and used documents

in connection with the order, submitted by the Employer, the logo and text on co-financing, in accordance with

with the Principles of project promotion for beneficiaries of the Operational Program Infrastructure and Environment

2014-2020.

8. The Contractor shall provide the Employer within 10 days of signing the contract for approval,

Quality Assurance Program.

9. The Contractor shall provide the Employer within 2 days prior to the date of signing the contract

photocopied copies of the rights of persons envisaged to perform on

independent technical function contracts.

10. The Contractor shall provide the Employer with statements of the Manager within 7 days of signing the contract /

Construction Managers with relevant attachments about taking independent technical functions on

given parts of the subject of the contract.
