1. The subject of the contract is the supply of medical equipment for "Poddębice Centrum Zdrowia" Sp. z o.o. in Poddębice, divided into 2 packages in quantities and assortment specified in Annex 2 to the SWZ – Price form and technical parameters of medical equipment.

2. The subject of the order includes:
