The subject of the contract is the provision of services in the field of transport (without medical supervision) of haemodialysis patients in a sitting position, including disabled patients (transport of patients with mobility limitations with the use of a wheelchair,
and in buildings not equipped with wheelchair lifts with the participation of a third party) from the place of residence to the Dialysis Station 10 of the Military Clinical Hospital with Polyclinic – SPZOZ in Bydgoszcz, and then transport of patients to their place of residence.
Determination of the subject matter and the size or scope of the contract:
1. The subject of the contract is the provision of services in the field of transport (without medical supervision) of patients with haemodialysis in a sitting position, including disabled patients (transport of patients with mobility limitations should be carried out with the use of a wheelchair,
and in buildings not equipped with wheelchair lifts with the participation of a third party) from the place of residence to the Dialysis Station 10 of the Military Clinical Hospital with Polyclinic – SPZOZ
in Bydgoszcz and then transporting patients to their place of residence.
2. The current number of patients covered by the order: 32 people. The number of people specified in the "weekly plan of transport for haemodialysis treatments" depends on the patient's health condition each time and may be subject to change during the term of the contract.
3. Number of dialysis sessions per month: 1 patient approx. 9 dialysis, and from 1 February 2024 up to 13 dialysis. Each patient receives dialysis three times a week.
4. Estimated number of treatments per month according to Appendix No. ... . The time between the arrival and drop-off of the patient can range from 4 to 6 hours. The time the Contractor's car is parked in front of the hospital should be limited to the necessary minimum, i.e. bringing and picking up the patient from the ward.
5. The services will be provided in a four-shift work system during the following hours*:
a. I shift from 7:00 a.m. to 11:30 a.m./ 12:00 p.m. (about 13 patients*);
b. 2nd shift from 12:30 p.m. to 5:00 p.m. / 5:30 p.m. (about 13 patients*);
c. III shift from 19:00 to 23:00/ 00:00 (about 13 patients* );
d. IV shift from 00:00 to 07:00 (as needed).
* - the maximum number of dialysis patients per shift.
6. Dialysis is performed continuously 6 days a week, i.e. from Monday to Saturday, except Sundays.
7. In case of emergency, it is necessary to transport the patient for the haemodialysis procedure outside the agreed schedule.
8. The local Dialysis Centre covers: Bydgoszcz - housing estates: Zawisza, Leśne, Śródmieście - border along Dworcowa - Śniadeckich - Gdańska - Kamienna - Armii Krajowa streets. Gmina Osielsko and Gmina Dąbrowa Chełmińska is bordered by the gminas of Osielsko.
9. The driver of the vehicle must have a driving licence of the appropriate category, a valid psychological examination, as well as a certificate of completion of first aid training. It is necessary for the driver to assist the patient in getting to the vehicle and to the dialysis unit, as well as to pick up the patient from the dialysis ward of the hospital and bring him to the apartment. Disabled patients in a wheelchair should be transported on a wheelchair, and in buildings not equipped with a wheelchair with the participation of a third party or a device for overcoming architectural barriers. Compliance with the Regulation of the Minister of Labour and Social Policy of 14 March 2000 on occupational health and safety in manual transport work.
10. The Contractor is obliged to provide: motor vehicles designed to carry more than 5 and not more than 9 people including the driver in a number that allows for timely provision of services which are the subject of this agreement, in accordance with the agreed schedule, in accordance with the agreed and applicable schedule, it is allowed to group patients
transport teams.
11. The vehicles used by the Contractor to transport patients must be adapted to transport disabled persons, including those in wheelchairs, in the necessary number.
