The subject of the contract is the insurance of persons, property and civil liability of the Municipality of Siemianowice Śląskie, its organizational units and the Volunteer Fire Brigade in Siemianowice Śląskie.

The order has been divided into two (2) lots.

Part 1: Insurance of persons, property and civil liability of the Municipality of Siemianowice Śląskie, its organizational units and the Volunteer Fire Brigade in Siemianowice Śląskie

Part 2: Motor insurance (OC, AC, NNW, Assistance) and motor insurance of vehicles to be scrapped

1) A detailed description of the subject of the contract in relation to Part 1 is included in Appendix No. 3a to the SWZ and in the Draft provisions of the contract (Appendix No. 4a to the SWZ).

2) The Contractor is obliged to perform the contract on the terms and conditions described in the Draft provisions of the contract (Appendix No. 4a to the SWZ).

3) The Contracting Authority shall not reserve the right to apply for the award of a contract only by Contractors referred to in Article 94 of the PPL, i.e. having the status of a sheltered workshop, social cooperatives and other contractors whose main purpose or main purpose of the activities of their organisationally separated units that will perform the contract is the social and professional integration of socially marginalized persons.

4) The Ordering Party reserves the right to reduce or exclude

in the scope of insurance specified in points IV and V of the Detailed description of the subject of the contract (Appendix No. 3a to SWZ).

In a situation where the Contracting Authority withdraws from the service, then the Contractor will not receive remuneration. At the same time, the Ordering Party guarantees that the amount to be paid

for the implementation of the subject of the contract, is the value given in the Contractor's offer for the scope of insurance specified in points I, II, III, VI and VII of the Detailed description of the subject of the contract (Appendix No. 3a to the SWZ).

5) In the description of the subject of the contract, the contracting authority did not take into account the requirements regarding accessibility for the disabled and design for all users referred to in Article 100(1) of the Public Procurement Law.

6) In the description of the subject of the contract, the Ordering Party did not specify the requirements for performance or functionality referred to in Article 101(1)(1) of the PPL.

7) The Contracting Authority does not require a label from the Contractor and does not indicate the applicable requirements of a specific label, referred to in Article 104 of the PPL.

8) The Contracting Authority reserves the right to award contracts referred to in Article 214(1)(7) of the PPL.

Scope of services to be provided:

the order will consist in repeating similar services as the basic order, i.e. property insurance against all risks, insurance of electronic equipment against all risks, civil liability insurance of the Ordering Party (the definition of the Ordering Party is given in the Detailed Description of the Subject of the Order - Appendix No. 3a to the SWZ) accident insurance (accident insurance) of volunteers, participants of cultural/sports/tourist events, insurance costs of medical treatment outside the Republic of Poland of employees of the Contracting Authority, Councillors of the City Council of Siemianowice Śląskie, accident insurance (NNW), new members of the Volunteer Fire Brigade, new members of the Youth Groups of the Volunteer Fire Brigade and accident insurance (NNW), debt collectors/executors, Inspectors of Investor Supervision and employees of the Waste Management Department, substantive employees of the Crisis Intervention Center and will be they are consistent with the subject of the basic contract carried out in the basic mode without the possibility of negotiations pursuant to Article 275 point 1 of the PPL Act and described in the Detailed description of the subject of the contract, constituting Appendix No. 3a to the SWZ.

The Ordering Party expects that the value of these services will not exceed PLN 150,000.00.

A contract may be awarded provided that there is at least one

from the following situations:

a) in the event of exhaustion of the amount referred to in § 2 clause 1 of the concluded contract,

b) if it is necessary to increase the amount of insured property or persons of the Ordering Party,

c) in order to maintain the continuity of the service in the period between the current contract and the conclusion of the contract in an open procedure.

The award of the contract will be preceded by a procedure under the

single-source, after negotiations with the Contractor.

the order will consist in repeating similar services as the basic orders, i.e. motor insurance of vehicles intended for scrapping, motor insurance of the Ordering Party's vehicles (OC, AC, NNW, ASSISTANCE) (the definition of the Ordering Party is given in the Detailed Description of the Subject of the Order – Appendix No. 3b to the SWZ) and they will be consistent with the subject of the basic contract carried out in the basic mode without the possibility of negotiations pursuant to Article 275 point 1 of the Act and described in the Detailed description of the subject of the order (Appendix No. 3b to the SWZ – in relation to Part 2).

The Ordering Party expects that the value of these services will not exceed PLN 30,000.00.

A contract may be awarded provided that there is at least one

from the following situations:

a) in the event of exhaustion of the amount referred to in § 2 clause 1 of the concluded contract,

b) if it is necessary to increase the number of insured vehicles of the Ordering Party,

c) in order to maintain the continuity of the service in the period between the current contract and the conclusion of the contract in an open procedure.

The award of the contract will be preceded by a procedure under the

single-source, after negotiations with the Contractor.
