Scope of construction works:

-demolition of existing concrete culverts

-manufacture of excavations with stability protection and dehydration

-fortification of the bottom and slope of the creek bed with stone overhead

-construction of a new object -corrugated sheet with oval cross-section

-adaptation of commuting and adjusting ditches

4. Lights of 2.96 x 2.00m. The length of the lower edge of the through structure of the object is 14.00m, and the upper 9.96m

ch2 culvert with light 3.35 x 2.19m.The length of the lower edge of the structure through the object is 12.00m, and the upper 9.06m.

The procedure shall be carried out in a mode: the unrestricted invitation to tender the Contracting Authority shall apply the so-called 'reverse' procedure referred to in Article 139 (1) to (4) of the PZP.

The Contracting Authority informs that it does not provide for the possibility of awarding the contracts referred to in Article 214 (1) of PKT 7 of the PZP Act

3.4The responsibilities of the investor supervision will include:

(a) acting in the interests and in favour of the Contracting Authority, representing the Contracting Authority on the construction site by checking compliance with its implementation with the design, regulations and technical knowledge rules,

(b) verifying the quality of construction works and built-in construction products and, in particular, preventing the use of construction products defective and not authorised for use in construction,

(c) checking and receiving construction works undergoing cover or disappearing within 3 days of the date of notification of the need for receipt by the construction manager,

(d) participation in technical trials and receipts and participation in final and warranty reception activities,

(e) confirming the elements of the works actually performed and the removal of defects,

(f) issuing the Contractor of Works - to the manager of construction commands, confirmed by the entry in the construction log, concerning:

-removal of irregularities or threats,

- request from the construction manager to make amendments or re-perform faulty works,

-halt further construction works in case their continuation could cause a threat or cause unacceptable incompatibility with the project.

(g) agreeing with the Contracting Authority the necessity of the performance of construction works - additional, complementary and replacement, carrying out the object of additional and complementary works. Without the Contracting Authority's consent, the Contractor (Supervisor) shall not be authorised to issue the contractor of the works a command to perform the above works,

(h) checking and verifying the cost of the contractor for additional, complementary and replacement works,

(i) controlling the course of the implementation of the construction works and, in the event of an irregularity, the occurrence of appropriate applications to the Contracting Authority,

(j) control the correctness of keeping the construction log and making entries therein stating all the circumstances relevant to the proper performance of the works,

(k) checking the quality and completeness of the post-built documentation,

l) controlling construction settlements

(m) to participate in the carrying out of trials, measurements and checks carried out in connection with the investment

n) verification of the works performed, notification to the contractor of the defects detected and certification of the removal of defects by the contractor, as well as establishing the type and scope necessary for the execution of corrective works

3.5The Supervisory Supervisor shall be obliged to adjust the working hours to the Contractor of Construction Works, whereby the Contracting Authority requires full disposition and inspection at the site of the construction works. The Contracting Authority reserves the right to call the Supervisor to the construction site if necessary.

3.6Insp. The supervision shall be obliged to submit a declaration of conformity of the works performed with the scope provided for in the contract with the Contractor and technical regulations, at the latest at the date of receipt of the works.

3.7The Contracting Authority shall not specify the employment requirements by the Contractor or Subcontractor on the basis of the employment contract of persons performing the performance of the contract indicated by the Contracting Authority, if the performance of these activities consists in the performance of the work in the manner laid down in Article 22 (1) of the Act of 26 June 1974 - Labour Code (text: Journal of Laws of 2020 item 1320 as amended). 4.2Required by the Ordering Party for the performance of the order:

-commencement on the date of signing the contract by the works contractor,

-completion on the date of signing the final acceptance protocol of works with the contractor of construction works

-and also during the warranty period applicable to the Contractor of construction works (min. warranty period for construction works required by the Ordering Party is 36 m-cy)

Information about the means of electronic communication by which the contracting authority will

communicated with the contractors

3.Communication between the Contracting Authority and the Contractors in this proceeding shall be carried out using the

purchase (hereinafter: “Platform”): https://platformazakupowa.pl/pn/lasy_kanczuga or by means of email:

kanczuga@krosno.lasy.gov.pl, subject to making an offer, ONE, as well as other statements or

documents submitted with the offer are made only using the Platform.

1) Communication between the Contracting Authority and the Contractors in particular the submission of statements,

requests (other than the Offer and JEDZ), notices and the transmission of information takes place

electronically via a dedicated form available on the Platform at

the procedure concerned. In correspondence related to this proceeding, the Contracting Authority and

Contractors use the number of the announcement published in the Public Procurement Bulletin

or the procedure number indicated in the SWZ.

2) The Ordering Party will also communicate with the Contractors via e-mail —

using the e-mail address of the Ordering Party and the e-mail address of the Contractor indicated in the offer form.

4.Technical and organisational requirements for, in particular, logging, submission of tenders,

submitting requests for clarification of SWZ content, sending and receiving electronic documents,

electronic copies of documents and statements and other information provided by using them

are described in the Instructions for Use of the Platform https://platformazakupowa.pl/strona/45-instrukcje

under the “Instructions for Contractors” tab and the Terms of Use of the Platform, available under

5.Limit the volume of files or packaged folders in terms of the entire offer, statements about

not subject to exclusion and fulfilment of the conditions of participation in the proceedings, as well as other statements

or documents submitted with the offer are 10 files or packed folders,

maximum size of a single file of 150 MB. During communication, the file size is

a maximum of 500 MB.

6.For the date of submission of the tender, ONE, as well as other statements or documents submitted with the offer, the date of the tender shall be accepted

their submission on the Platform.For the date of submission of applications, notices, documents, statements or copies thereof and other information shall be accepted

the date of their transfer to the e-mail address of the Contracting Authority indicated in point 3 or submission to

Technical requirements enabling the use of the Platform provided by the Contracting Authority

1) Permanent Internet access with a guaranteed bandwidth of not less than 512 kbps,

2) PC or MAC class computer with the following configuration: 2 GB Ram memory, Intel IV 2 processor

GHZ or its newer version, one of the operating systems — MS Windows 7, Mac Os x 10 4, Linux,

or their newer versions,

3) installed any web browser, in the case of Internet Explorer minimally

electronic signature with a qualified electronic signature
