The subject of the contract are successive deliveries of pharmaceutical products to SP ZOZ in Myszków.

The delivery specification is contained in Annex 2 to this SWZ.

The order is divided into 2 packages (parts) listed in Annex 2 to SWZ.

Each Contractor may submit a tender in the range of one or any number of packages selected, while within the package the offer must be complete (i.e., it must cover all items), otherwise it will be rejected.

When describing the subject of the order, the Contracting Authority specifies the assortment in detail in accordance with the needs of the Hospital, however, he also allows an equivalent product by which he means a preparation of the same use and chemical composition, form and route of administration, as described in Annex No 2 (“Form assortingly - priced”) unless otherwise noted.

The quantities given in the assortment and price forms refer to the period of 8 months.
