Health services. Provision of medical and nursing care to patients in their own home as an alternative to a hospital admission. The external Provider will work with Emergency Department (ED) consultants to identify patients who would normally be admitted to an acute hospital bed and manage their condition at home. The patient will receive the treatment they would have had in hospital and this may include, monitoring, IV antibiotics, medications and rehabilitation therapies. The service will not compromise the delivery of the 4 hour A&E target neither will it duplicate an existing provision, health or social care.
The external Provider should access technology and tele-medicine to facilitate and support safe provision of care. The wider multidisciplinary team of the Provider should include allied health professionals, pharmacists and health care assistants.
