The subject of the contract is the delivery of gift cards (hereinafter referred to as cards) for the employees of the Teatr Wielki – Polish National Opera in the form of electronic prepaid bearer cards with a PIN code or confirmed by a signature in the maximum number of:
- for 15 employees with a value of: PLN 2,200.00 for the amount of PLN 33,000.00,
- for 89 employees with a value of: PLN 2,000.00 for the amount of PLN 178,000.00,
- for 364 employees with a value of: PLN 1,750.00 for the amount of PLN 637,000.00,
- for 311 employees with a value of: PLN 1,650.00 for the amount of PLN 513,150.00.
A total of up to 779 cards with a total value of PLN 1,361,150.00 (say: one million three hundred and sixty-one thousand one hundred and fifty, 00/100).
The Ordering Party reserves the right to reduce the order by a maximum of 30 cards, regardless of their value in relation to their total maximum number.
A detailed description of the subject matter of the contract is presented in Appendices 1 and 5 to the Terms of Reference.

The Ordering Party reserves the right to reduce the order by a maximum of 30 (thirty) cards, regardless of their value, in relation to their total maximum number referred to above. In such a case, before concluding the contract, the Contracting Authority shall provide the Contractor with information on the number and value of the cards by which the order is reduced by e-mail. In this case, the Contractor's remuneration (offer price) will be reduced by the value of the cards by which the order has been reduced.
2. A detailed description of the subject matter of the contract is presented in Appendix 1 to the Terms of Reference entitled "Description of the subject matter of the contract" (OPZ). The scope and manner of performance of the contract are specified in the draft provisions of the agreement constituting Appendix No. 5 to the Terms of Reference.
