1. The subject of the contract is the DELIVERY OF DISPOSABLE EQUIPMENT in accordance with the description and requirements contained in Annexes No. 2 to the Specification (in the assortment and price forms).
2. The subject of the contract has been divided into 17 packages (tasks).
3. The Contracting Authority allows submission of partial bids for individual packages (tasks, lots).
4. In a particular package, i.e. in each assortment and price form, all items must be included. Lack of any assortment item in a given Appendix No. 2 will result in rejection of a given tender offer.
5. Tenders may be submitted in one or more or all lots.
6. The deadline for the performance of the subject of the contract (contract) will take place for a minimum period of 24 months from the date of conclusion of the contract or until the amount constituting the contractual remuneration is exhausted.

GYNECOLOGICAL PACKAGE AND ASSORTMENT (Annex No. 2.1.)The package includes, among others, such as: disposable vaginal speculums, cytological toothbrushes, powdered casings – in accordance with the description and quantities specified in Annex 2.1.
PACKAGE X ELECTRODES (Attachment No. 2.10.)The package includes ECG electrodes for resting tests, monitoring and diagnostics and electrodes – as described and amounts specified in Annex 2.10.
PACKAGE XI GENERAL MEDICAL EQUIPMENT (Attachment No. 2.11.)The package includes, among others, such as: compression stases, enema sets, T-type drains – KEHR, interchangeable blades for scalpers, anatomical and surgical tweezers, umbilical cord crimpers, finger rails, stabilizers for endotracheal tubes, mouthpieces for gastroscopy, protective glasses for phototherapy for newborns, bottles for long-term wound suction, umbilical cord clamping clamp, gels for ultrasound and EEG, medication glass, medical connectors, laryngological mirrors, ear speculums, nasal speculums, laryngological spatulas, containers, feeding bibs, vomit bags, aprons
made of polyethylene – as described and in the quantities specified in Annex 2.11.
PACKAGE XII NON-WOVEN ASSORTMENT OF DISPOSABLE (Attachment No. 2.12.)The package includes, among others, such as: shorts / shorts for colonoscopy, a set of medical bedding, disposable washcloths, tissue paper pads, sheets j. u., shoe covers, panties for tests, – according to the description and quantities specified
in Appendix No. 2.12.
PACKAGE XIII URINE SAMPLING ACCESSORIES (Attachment No. 2.13.)The package includes sterile containers with PP for urine and tubes for urinalysis – as described and amounts specified in Annex 2.13.
PACKAGE XIV TEST STRIPS FOR GLUCOSE METERS (Attachment No. 2.14.)The package includes test strips for Gluco MAXX glucose meters in the hospital's equipment – as described and in the quantities specified in Annex 2.14.
PACKAGE XV PUMPING AND STORAGE EQUIPMENT (Attachment No. 2.15.)The package includes a set for collecting food for Symphomy breast pumps and bottles for the above-mentioned sets – as described and quantities specified in Annex 2.15.
PACKAGE XVI LOTIONS FOR RINSING THE BLADDER (Annex No. 2.16.)The package includes an assortment for rinsing the bladder – in accordance with the description and quantities specified in Annex 2.16.
PACKAGE XVII ASSORTMENTEWNT FOR THE KIT FOR TRANSPORTING NEWBORNS WITH CPAP (Attachment No. 2.17.)The package includes an assortment for the CPAP ATOM newborn transport kit (CPAP adapter for ventilator, humidifier chamber with automatic water filling, patient system with generator – as described
and the quantities specified in Annex 2.17.
PACKAGE II IDNTIFICATION WRISTBANDS (Annex No. 2.2.)The package includes identification bands for newborns, infants and adults – as described and quantities specified in Annex 2.2.
PACKAGE III CLIPPER, BLADES FOR CLIPPERS (Annex No. 2.3.)The package includes clippers and clipper blades – as described and quantified in Appendix No. 2.3.
PACKAGE IV PLASTIC CONTAINERS FOR STORAGE AND TRANSPORT OF SURGICAL SPECIMENS (Annex No. 2.4.)The package includes plastic containers filled with 10% formalin solution intended for safe storage and transport of tissue materials – as described and quantified in Annex 2.4.
PACKAGE V PLASTIC CONTAINERS FOR BIOPSY/HISTOPATHOLOGICAL SPECIMENS (Annex 2.5.)The package includes plastic containers for biopsy/histopathological samples with formalin enclosed in a cap – as described and quantified in Annex 2.5.
PACKAGE VI ACCESSORIES FOR SPIROMETER "PNEUMO RS 220 V" (Annex No. 2.6.)The package includes an assortment for the spirometer equipped with tut. hospital, among others: pneumotachograph, plastic mouthpiece, nose clip and pneumotachograph hose – as described and quantities specified in Annex 2.6.
PACKAGE VII PAPERS FOR ECG, CTG AND ULTRASOUND VIDEOPRINTERS (Attachment No. 2.7.)The package includes such assortment as: thermal paper for ECG machines, CTG and ultrasound videoprinters – in accordance with the description and quantities specified in Annex No. 2.7.
PACKAGE VIII NEEDLE LANCETS (Annex No. 2.8.)The package includes sterile automatic haematological lancing devices – as described and quantities specified in Appendix No. 2.8.
PACKAGE IX DISPOSABLE BRUSHES FOR CHIRORGIC HAND WASHING (Attachment No. 2.9.)The package includes dry, sterile disposable toothbrushes for surgical hand washing (without detergent) – as described and in the quantities specified in Annex 2.9.
