The order was divided into two Parts. Part 1 of the order - the provision of specialized care services for people with aphasia. The subject of the contract is the provision of specialized care services for people with aphasia from Gdynia. The scope of the Service includes learning and developing the skills necessary for independent living and preparing to undertake specialized therapeutic support.

Part 2 of the order is to provide specialized care services for children and for adults on the autism spectrum, including those with Asperger's syndrome. The subject of the order is the provision of specialized care services for children and adults on the autism spectrum, including Asperger's syndrome from Gdynia.

Detailed description of the item of the order is specified in the Order Terms Specification (hereinafter SWZ)

Cz.1 the provision of specialized care services for persons with aphasia from Gdynia, as defined in the Law of 12 March 2004 on social assistance (Journal of Laws of 2020 item 1876, as amended) and section. Minister of Social Policy of 22 September 2005 in the disputes of specialized care services (Journal of Laws 2005 No. 189 item 1598 as amended). The scope of the Service includes learning and developing the skills necessary for independent living and preparing to undertake specialized therapeutic support. The service will be provided for 45 people, totalling 1440 hours during the contract (i.e. an average of 72 hours per person). Persons for the benefit of which the Service will be provided will be indicated by the Order.. The service will be carried out on working days from Monday to Friday from 7:00 to 19:00 (it is permissible to provide services on Saturday if all interested persons agree to do so). Under the term of the hour, he understands the clock hour i.e. 60 minutes. The Contracting Authority reserves the possibility of reducing the expected hour dimension of services provided, in particular depending on the number of applicants for the award of the indicated form of aid or the incomplete security of the funds for the implementation of the services by the Pomeranian Voivode. The above number of hours of services is max., while min. the hour dimension of services provided during the period of execution of the contract is 700 hours. Contractor is not entitled to claim the order of the full number of hours. Contractors will only be entitled to the remuneration resulting from the completed services and no it will make claims on the implementation of the remainder. Detailed information on the order and the method of its implementation, specifies zal 1 to SWZ - Gdyński standard of specialized care services provided at the place of residence.Individualized range of services for each person covered by them each time will be determined by the Ordering Party in by administrative procedure and forwarded in writing in the framework of an agreement concluded with the elected Contractor. The Contractor shall be obliged throughout the period of execution of the contract to provide the persons providing care services with rates of hour. gross remuneration which, regardless of the form of employment, shall not be less than 37 PLN /1h.

1. The description of the object of the order for Cz 1 and Cz 2 is set out in the SWZ in the contract template, which is the order number 2.1 for Cz 1 and the number 2.2. for Cz 2 to SWZ. The Contracting Authority shall not reserve the obligation to personally perform key parts of the order by the Contractor. The Contracting Authority shall not reserve the possibility of applying for a contract only by the Contractors referred to in Article 94 of the Pzp Act. The Contractor may entrust execution of part of the contract to the Subcontractor The Contracting Authority shall not provide for the obligation of the Contractor to carry out a local vision and to check the documents necessary for the performance of the contract available on the spot with the Contracting Authority. The Order. does not allow the possibility of submitting a variant offer, Article 92 of the Pzp Act. The Order. does not provide for the conclusion of a framework agreement and the establishment of a dynamic purchasing system and does not provide for the selection of the most advantageous offer using electronic auction, does not provide for the award of contracts pursuant to Article 214 (1) (8) of the Law Pzp, does not allow the submission of tenders in the form of electronic catalogs or the attachment of electronic catalogues to the offer, in the situations set out in Article 93 of the Pzp Act. The settlement of the order is made in PLN. The Contracting Authority does not provide for the reimbursement of the costs The Order. does not provide for the granting of advances for the performance of the contract. The Order. does not require employment by the Contractor or the Subcontractor, on the basis of the employment relationship, of the persons performing the activities during the execution of the contract.

Cz 2 - the provision of specialized care services for children and adults on the autism spectrum, including Asperger's syndrome from Gdynia, as defined in the law of 12 March 2004 on social assistance (t. Journal of Laws of 2020 item 1876, as amended) and the regulation of the Minister of Policy Community of 22 September 2005 in specialised care services (Journal of Laws 2005. No 189, item 1598, as amended) .Services will be provided in a dimension of 18,000 hours during the duration of the contract (on average of 1500 hours), to 45 persons, indicated by the Ordering., on the day of the day from Monday to Friday from 7:00 to 19:00. It is permissible to provide the Services on Saturday, as long as both parties agree to it. An hour understood as a clock hour i.e. 60 minutes. The Order reserves the possibility of reducing the projected dimension of the services provided, in particular depending on the number of applicants for the award of the specified form of aid or the incomplete security of funds for the implementation of the Services by the Pomeranian Voivode. The above number of hours of services is poppy., while min. the dimension of h provided Services during the period of execution of the order. It is 11,000 hours. Contractors are not entitled to claim the order of the max. number of hours. The Contractor will only be entitled to the remuneration arising from the completed Services. Detailed inf. on the procurement and the method of its implementation, defines zal 1 to SWZ - Gdynia standard of specialized care services provided at the place of residence. The individualized scope of the Services for each person covered by them shall be determined by Zam. by administrative procedure and transmitted in writing to the Contractor. The Contractor will be obliged to provide the persons providing the Service with rates of hour. gross remuneration which, regardless of the form of employment, will not be less than 37 PLN /1h. The Contractor will be required to secure the therapy materials necessary for the implementation of the individual scope of the Services., keep the documentation specified in the contract, and to provide the Orders. documentation allowing to determine the regularity of the implementation of the contract. The Contractor will be required to have premises in Gdynia for coordination, settlement and contact in matters of services provided. The premises should have a direct telephone number, access to email and be available to stakeholders at least between 8:00 and 16:00 on working days.
