1. The subject of the order is the supply of diagnostic, surgical and personal protection gloves.

3. The Employer in Annex 2 to the ToR requires the Contractor to provide, in addition to the price, the catalog code number, trade name and manufacturer, the subject of the contract offered by the Contractor.

4. The period of validity of the offered subject of the contract may not be less than 12 months from the date of each delivery.

5. The offered object of the contract must be authorized for trading and use in the territory of the Republic of Poland in accordance with the requirements of the Act of 20.5.2010 on medical devices (Journal of Laws of 2019, item 175) and European Union directives in accordance with from art. 26 of the Act of 20.5.2010 on medical devices (does not apply to part No. 4, items 1-4).

5. The Awarding Entity provides for the selection of the best bid using electronic auction, in lot 1.

6. The subject of the contract will be delivered at the cost, risk and transport of the Contractor.

7. The Contractor is obliged to perform the contract on the terms and conditions described in the model contract, constituting Annex 5 to the ToR.

3. The Employer in Annex 2 to the ToR requires the Contractor to provide, in addition to the price, the catalog code number, trade name and manufacturer, the subject of the contract offered by the Contractor.
