The subject of the contract are the supply of equipment for medical use for the Provincial Specialist Hospital in Legnica - described in detail in Annex 2A to the SWZ (assortment and price form), which is an integral part of the specification of the terms of the contract, grouped into 2 parts.
Part 1 X-RAY APPARATUS WITH C-ARM - delivery, installation, commissioning, electrical safety test and training1 The subject of the contract are the supply of equipment for medical use for the Provincial Specialist Hospital in Legnica X-RAY APPARATUS WITH C-ARM - delivery, assembly, commissioning, electrical safety test and conducting training - described in detail in Annex 2A to the SWZ (assortment and price form), which is an integral part of the tender specifications.
2. The Ordering Party requires that the offered medical devices:
- bear the CE marking and, according to their class, current notified body certificates and/or declarations of conformity;
- met the conditions for admission to trading in accordance with the Act on medical devices of 07 April 2022.
- correspond to the description of the subject of the contract contained in Annex 2A to the SWZ
3. Conditions for the execution of the contract are also set out in the draft agreement in Chapter XX of the SWZ.
Part 2 - A set of equipment for the Olympus endoscopic tower owned by the Ordering Party - delivery, assembly, commissioning1. The subject of the contract are the supply of equipment for medical use for the Provincial Specialist Hospital in Legnica - A set of equipment for the endoscopic tower of the manufacturer Olympus being the property of the Ordering Party - delivery, assembly, commissioning of the assortment and price form described in detail in Annex 2A to the SWZ, which constitutes an integral part of the contract specification.
2. The Ordering Party requires that the offered medical devices:
- bear the CE marking and, according to their class, current notified body certificates and/or declarations of conformity;
- met the conditions for admission to trading in accordance with the Act on medical devices of 07 April 2022.
- correspond to the description of the subject of the contract contained in Annex 2A to the SWZ
3. The conditions for the performance of the contract are also specified in the draft contracts in Chapter XX of the SWZ.
