Providing support to Service Users and their relatives/carers from hospital admission to 3 weeks after discharge to ensure safe and timely discharge, following Home First principles, and reducing bed shortages in secondary care.
Home from Hospital Discharge Services for Northampton General Hospital Trust (NGH). The purpose of the service is to support Service Users and their relatives / carers from the start of their hospital stay up to 3 weeks post discharge. This service would support safe and timely discharge when there is no ‘Reason to Reside’ in a hospital bed and the Home First principles. This service will also help to reduce bed shortages within secondary care through improved patient flow.
