The subject of the order is successive deliveries of medical gloves for the needs of the Provincial Hospital in Kielce. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference). Information on the terms of delivery, guarantee and remuneration is contained in Appendix No. 3 to the Terms of Reference – draft provisions of the public procurement agreement. The Contracting Authority allows for the possibility of submitting partial offers for individual packages from 1 to 9 (none of the packages is subject to division). In the event that the bid of one contractor, submitted for more than one package of this procedure, turns out to be the most advantageous, the Contracting Authority shall conclude a single contract for the performance of the public contract.

Package No. 1 - Nitrile gloves for rail-mounted and wall-mounted mounts. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference).

Package No. 2 - Diagnostic nitrile gloves. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference).

Package 3 - Nitrile gloves designed for procedures with a high risk of infection. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference).

Package No. 4 - Nitrile gloves with extended cuff for high-risk procedures. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference).

Package No. 5 - Disposable vinyl gloves. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference).

Package No. 6 - Latex surgical gloves. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference).

Package No. 7 - Surgical gloves, powder-free, sterile. A detailed description of the subject of the contract and quantities is specified in the assortment and price form (Appendix No. 2 to the Terms of Reference).
