The subject of the contract is the provision of transport services for hemodialysis patients and sanitary transport of blood and its components for the Provincial Hospital Complex in the following scope: Tasks No. 1 A. transport of hemodialysis patients from their place of residence to the Dialysis Centre of the Provincial Hospital Complex in Kielce within the administrative borders of the city of Kielce and back home. B. transport of hemodialysis patients from their place of residence to the Dialysis Centre of the Provincial Hospital Complex in Kielce outside the administrative borders of the city of Kielce and back home. Tasks No. 2: Sanitary transport - collection and delivery of blood and its components with documentation from the Blood Donation Station in Kielce, 66 Jagiellońska Street to the Transfusion Immunology Laboratory and from the Transfusion Immunology Laboratory to the Department of Infectious Diseases at 7 Radiowa Street and to the Świętokrzyskie Paediatric Center II Department of Paediatrics Department of Paediatric Oncology and Hematology Pavilion G at 3a Artwińskiego Street. Pursuant to Article 5(33a) of the Act of 27 August 2004 on health care services financed from public funds (Journal of Laws 2022.2561), sanitary transport is the transport of persons or biological materials and materials used to provide health services requiring special transport conditions. A detailed description of the subject of the contract is set out in Annex 2 to the SWZ – assortment and price form, Annexes 2a and 2b to the SWZ (list of places from which patients will be transported to the WSzZ Dialysis Station on particular days of the week) and Appendix No. 3 – draft provisions of the public procurement contract. Pursuant to Article 95(1) of the Act, the contracting authority requires that throughout the period performing the service, the subcontractor employs persons providing the service used for the transport of dialysis patients or sanitary transport of blood on the basis of an employment relationship, provided that the activities performed consist in performing work in the manner specified in Article 22 § 1 of the Act of 26 June 1974 – Labour Code (Journal of Laws 2022.0.1510, t. j.). The number of employees necessary to perform the subject of the contract shall be determined by the contractor, taking into account the deadline for performance and the complexity of the subject of the contract.

B. transport of hemodialysis patients from their place of residence to the Dialysis Centre of the Provincial Hospital Complex in Kielce outside the administrative borders of the city of Kielce and back home. Hours of arrival of hemodialysis patients for individual shifts: 1) I shift – 7:15 – 13:00 2) II shift – 13:00 – 18:30 3) III shift – 18:30 – the last dialysis session ends around 00:00.

Tasks No. 2: Sanitary transport - collection and delivery of blood and its components with documentation from the Blood Donation Station in Kielce, 66 Jagiellońska Street to the Transfusion Immunology Laboratory and from the Transfusion Immunology Laboratory to the Department of Infectious Diseases at 7 Radiowa Street and to the Świętokrzyskie Paediatric Center II Department of Paediatrics Department of Paediatric Oncology and Hematology Pavilion G at 3a Artwińskiego Street. The Contractor will each time collect containers and cartridges necessary to transport blood and its components directly from the Transfusion Immunology Laboratory of the Provincial Hospital Complex in Kielce, 45 Grunwaldzka Street: a. thermal container (termotorba) – a container used to transport blood or blood products, ensuring the possibility of maintaining proper transport conditions, b. ice cartridge – cartridge or other factor enabling the maintenance of the appropriate temperature in the container thermal (termotorbie) for the transport of blood or frozen plasma, c. warm cartridge - an insert or other factor that allows to maintain the appropriate temperature in the thermal container (termotorbie) for the transport of platelets.
