Round-the-clock provision of medical transport services for patients with a complex team and a simple team within the Hospital, in the city of Opole and outside the city of Opole for a period of 12 months (guaranteed period). As part of the concluded contract, the Contracting Authority provides for the possibility of applying the right of option consisting in the possible extension of the contract with the Contractor for a further period of 12 months (option I), and another period of 12 months (option II), and another period of 12 months (option III) while maintaining the same conditions and scope of the subject of the contract as in the guaranteed contract. The maximum number of extensions is three, which means that the maximum duration of the contract in the case of exercising the right of option can be a total of 48 months. A detailed description and scope of the subject of the contract is specified in the contract template (with attachments) constituting Annex No. 1 to the SWZ and in the PRICE FORM constituting Annex No. 2 to the SWZ, which will constitute an attachment to the concluded contract.
The Contractor is obliged to implement the subject of the contract entitled "Round-the-clock provision of medical transport services for patients from:
(1) a complex team, and
2) simple syndrome
within the Hospital, in the city of Opole and outside the city of Opole", 7 days a week 24 hours a day (including non-working days, Sundays and holidays), in particular in the field of medical transport of patients requiring medical supervision during transport, including:
a) medical transport of the patient to another unit for the purpose of agreed hospitalization or consultation,
b) medical transport of the patient to the diagnostic laboratory outside the Provincial Hospital in Opole,
c) medical transport of the patient within the Provincial Hospital in Opole,
d) medical transport of a patient requiring transport in a lying or sitting position using sanitary transport with a complex or straight syndrome - from the place of residence to the Dialysis Station of the Provincial Hospital in Opole at 64a Katowicka Street, and
e) medical transport of a patient requiring transport in a lying or sitting position using sanitary transport with a complex or simple syndrome - after the end of dialysis from the Dialysis Station of the Provincial Hospital in Opole at 64a Katowicka Street to the indicated place of residence.
