1. The subject of the contract is the supply of various reagents along with the lease of analysers, specified in parts for the Clinical Department of Laboratory Diagnostics of the Fryderyk Chopin University Clinical Hospital in Rzeszów, including:

a) Reagents and media for the culture of Mycobacterium tuberculosis together with the lease of a fluorometric analyser for the culture and determination of drug susceptibility of Mycobacterium tuberculosis - Part No. 1;

b) Reagents and consumables for research in the field of genetics together with the lease of an analyser for genetic tests of molecular identification of microorganisms - Part No. 2;

c) Reagents, control and consumables for the determination of electrolytes in serum, plasma and whole blood, together with the lease of an analyser for the determination of electrolytes (Na, K, Cl) - Part No. 3;

(d) Reagents, control materials and calibrators, consumables for the determination of specific proteins together with the lease of an analyser for the determination of specific proteins — Part No. 4.

The subject of the contract is the supply of reagents and media for the culture of tuberculosis bacilli together with the lease of a fluorometric analyser for the culture and determination of drug susceptibility of tuberculosis bacilli for the Clinical Department of Laboratory Diagnostics of the Fryderyk Chopin University Clinical Hospital in Rzeszów.

A detailed description of the subject matter of the contract, the estimated quantity and requirements are presented in: Price and offer forms for Lot No. 1 (Appendices No. 1 and 2 to the Terms of Reference), Summary of parameters required for Lot No. 1 (Appendix No. 4 to the Terms of Reference), which are an integral part of the Terms of Reference.

3. The scope of the order includes the delivery of the order items in the quantities specified in the Price and Offer Forms, which are an integral part of the SWZ for a period of 36 months.

4. The Contracting Authority shall not allow the submission of equivalent tenders.

5. Delivery of the items of the order together with the lease of the analyser for 36 months from the date of conclusion of the contract;

The Ordering Party allows for the possibility of changing the term of the contract in the event of failure to complete the entire subject of the contract during the contract period as a result of the implementation of lower orders (does not apply to lease). An amendment to the contract requires an annex, however, the Contracting Authority unilaterally notifies the Contractor about the proposed extension of the contract before the expiry of the term for which it was concluded.

A detailed description of the subject matter of the contract, the estimated quantity and requirements are presented in the following forms: Price and offer forms for Lot No. 3 (Appendices No. 1 and 2 to the Terms of Reference), List of parameters required for Lot No. 3 (Appendix No. 4 to the Terms of Reference), Conditions of servicing the analyzer for Lot No. 3 (Appendix No. 6 to the Terms of Reference), which are an integral part of the Terms of Reference. .

3. The scope of the contract includes the delivery of the items specified in the Price and Offer Forms, which are an integral part of the Terms of Reference, for a period of 24 months.

4. The Contracting Authority shall not allow the submission of equivalent tenders.

5. Delivery of the items of the order together with the lease of the analyzer for 24 months from the date of conclusion of the contract;
