1. The subject of the contract is a comprehensive patient nutrition service, i.e. the provision of food services in the field of:

(a) the preparation and delivery of all-day meals for patients of the Provincial Psychiatric Hospital in Złotorya, 9 Hospital Street;

(b) preparation and delivery of all-day meals for patients of the Psychiatric Center and Odw. Op. Zdr. in Legnica, at 81 Chojnowska Street;

(c) the provision of nutritional rations for patients of the Psychiatric Daily Department in Legnica;

d) lease of premises and kitchen appliances of the Provincial Psychiatric Hospital in Złotorya, at 9 Hospital Street.

2. Estimated quantities:

(a) Estimated number of separates:

— Provincial Psychiatric Hospital in Złotorya — about 153,957

— Addiction Treatment Branch in Legnica — about 30,167

(b) Estimated quantity of rations during the duration of the contract:

— Psychiatric Daily Branch in Legnica — about 16,037

1. The contractor is obliged to:

(a) daily preparation and delivery of meals (breakfast, lunch, dinner) to the wards of the Provincial Psychiatric Hospital in Złotorya, 9 Hospital Street:

• breakfast, around 8:00 — 8:30

• dinner, about 13:00 — 13:30

• dinner, around 17:00 — 17:30

(b) daily preparation and delivery of meals to the Psychiatric and Rehab Health Care Centre in Legnica, at 81 Chojnowska Street:

• breakfast + ration, about 8:30

• dinner, about 13:30

• dinner, around 17:30

(c) the preparation of meals taking into account the diets and additives used in the nutrition of the sick:

• basic diet • easy digestible diet

• diabetic diet • vegetarian diet

• hepatic diet • possibly different depending on your needs

The most commonly used diets are an easily digestible diet, a diabetic diet and a liver diet. The amount of patients using diets varies widely, typically it is about 450 people per month.

The above-mentioned diets are not a closed catalogue, as other diets are allowed, not mentioned above, and dictated by the specific needs of patients.

(d) delivery on weekdays, nutrition rations to the Psychiatric and Rehab Health Care Centre in Legnica, at 81 Chojnowska Street.

e) the preparation of paid meals in the form of “takeaway” for the staff of the Hospital (and their families). Lunch is ordered to be phone/in person from 7:00 to 11:00 hours. Number of lunches sold by the current Contractor for employees and their families: approximately 50 meals per month.

(f) transport together with delivered meals, possible small shipments to and from the Psychiatric and Rehab Health Care Centre in Legnica, at 81 Chojnowska Street.

2. The Contracting Authority provides for orders, in accordance with Article 214 (1) (7) uPzp, up to 10% of the sub-order.
