1. The subject of the contract are: Supply of small disposable medical equipment 2. The Ordering Party divides the subject of the contract into the following parts: 2.1. task no. 1 – Cannulas, instruments, ports, 2.2. task no. 2 – tbc syringes and insulin, 2.3. task no. 3 – Three-piece syringes, 2.4. task no. 4 – Safety needles, 2.5. task no. 5 – Syringes, needles, small equipment, 2.6. task no. 6 – Catheters, tubes, catheters, drains, 2.7. task no. 7 – Miscellaneous containers, 2.8. Task No. 8 – Medical art, filtry_podklady, materials consumable in electromedicine 2.9. Task No. 9 – Consumables for hemodynamic measurements, 2.10. Task No. 10 – Consumables for non-invasive respiratory support, 2.11. Task No. 11 – Disposable equipment for intensive care, 2.12. Task No. 12 – Airway obstruction equipment and skin staplers, 2.13. Task No. 13 – Disposable equipment used in Gastroenterology, 2.14. task No. 14 – Electromedical accessories and electrodes, 2.15. Task No. 15 – Single-use kits and equipment for splinting ureters. 2.16. task No. 16 – breast pump, breast pump set, 2.17. task No. 17 – Surgical films, 2.18. task No. 18 - Helicobacter detection test, 2.19. Task No. 19 – Biopsy instruments 2.20. Task No. 20 - Equipment for Hamilton devices 2.21. Task No. 21 - Bronchial tubes-1 2.22. Task No. 22 - Bronchial tubes-2 3. The Contracting Authority does not limit the number of parts for which the Contractor may submit a bid. This means that the Contractor may submit a bid for one/several/dozen/several dozen or all parts. 4. A detailed description of the subject of the contract is included in: 1) Annex 2 to the SWZ, i.e. in the assortment and price form, 2) the contract template – Annex 5 to the SWZ, which are an integral part of the SWZ. The requirements specified in at least one document constituting annexes to the SWZ shall be binding for the Contractor as if they were included in the entire documentation. 5. The quantities given in the assortment and price forms (they specify the basic order – without taking into account the options) are used only for the purpose of comparing offers. The final value of the subject of the contract shall be determined on the basis of the actual quantity of the contract, resulting from the actual demand of the Ordering Party, multiplied by the unit price, with the proviso that the minimum level of performance of the contract will not be less than 70% of its maximum value and the Contractor shall not have any claims in this respect. 6. In the description of the subject of the contract, the contracting authority has specified quality requirements relating to at least the main elements constituting the subject of the contract – therefore, it uses the option indicated in Article 246(2) of the PPL.
