The subject of the contract is the provision of internal cleaning services (part II). VI and VII orders) and external, including the maintenance of green areas (part II). VI orders) in the buildings of the Małopolska Voivodship Office in Kraków and Wieliczka. The services have been divided in the field of cleaning services into:

- material scope of internal and external cleaning of buildings and green areas, including the performance of economic works – basic (obligatory) scope,

- material scope of optional (additional) works – additional (non-obligatory) scope.

The subject of the contract in parts VI and VII are services related to the location of Krakow and Wieliczka.

1. The subject of the contract in part NO. VI are services related to the location – Krakow: a complex of buildings at Półłanki 78 Street, Szlak 73a Street, Elizy Orzeszkowa 7 Street; Wieliczka, 22 Parkowa Street

1) the building located at 78 Półłanki Street - consists of two parts (office building + warehouse hall and the building of the State Fisheries Guard (PSR)),

2) the building located at 7 Elizy Orzeszkowa Street is the building of the Archive of the Małopolska Voivodeship Office,

3) the building located at Szlak 73a Street - is a new, five-storey building (including one underground storey). The building is fully adapted to the needs of people with disabilities. A building under access control. Sliding entrance gate, opening towards the west. Fenced area. Part of the parking spaces paved and part sprinkled with small stones,

4) the building located at 22 Parkowa Street in Wieliczka is a two-storey building with a utility attic. The premises are used sporadically.

A detailed description of the subject of the contract and the conditions for the performance of the contract are specified in Annex 1 to swz – model contract with attachment – description of the subject of the contract "Detailed list of activities included in the scope of services together with a description of facilities, buildings and premises of the Małopolska Voivodship Office in Krakow" (OPZ) for all parts of the contract.

3. The amount intended to finance the contract in part II VI:

- basic order: PLN 754,448.45 gross

- order consisting in the repetition of similar services: PLN 75,444.85 gross

TOTAL PLN 829,893.31 gross

1. The subject of the contract in part VII are services regarding the location – Krakow: buildings at Kanonicza 24, Plac na Stawach 3 and Ujastek 7

1) the building located at 7 Ujastek Street - Małopolska Voivodship Office rents office space there. On average, 12 meetings of the Commission on Medical Events are held there 12 times a month,

2) a building located at 3 Plac na Stawach Street – this is a building in which only floors I and II belong to the Małopolska Voivodeship Office. In the part belonging to the MUW, about 10-20 people are admitted per day, depending on the application season,

3) a building located at 24 Kanonicza Street - a building in which the Department of Revalorization of Krakow Monuments and National Heritage of the Małopolska Voivodeship Office in Krakow has its headquarters.

3. The amount intended to finance the contract in part II VII:

- basic order: PLN 189,223.29 gross

- order consisting in the repetition of similar services: PLN 18,922.33 gross
