1. The subject of the contract includes group insurance of employees, spouses or life partners and adult children of employees of the entities listed in section I.

2. The Employer requires that the Employer (Employer's units) are not obliged to cover the losses of the Contractor acting in the form of a mutual insurance company by paying an additional premium, in accordance with Art. 111 section 2 of the Act of 11.9.2015 on insurance and reinsurance activity (Journal of Laws of 2018, item 999, as amended).

3. The contractor must have general (special) insurance conditions, hereinafter referred to as GTC, for all insurance specified in the subject of the contract.
