Delivery, assembly and commissioning of 815 solar installations, 2438 micro-photovoltaic installations, 295 installations with an air heat pump for the needs of hot water or central heating, and 133 installations with a biomass boiler - pellets intended for households under the program "Partnership project for the construction of renewable energy installations for communes of the Lesser Poland voivodship" on the private properties of residents of partner communes, divided into four parts.

PART I - delivery and assembly together with commissioning of 815 solar installations on private real estate of residents of partner municipalities: implementation of solar installations on private buildings or on land belonging to residents of partner municipalities by supplying installation elements and carrying out on-site assembly of solar installations together with commissioning and instruction. Three types of solar installations are envisaged, depending on their power and volume of storage heaters (sets), in quantities:

- Set I: 2 flat solar collectors, supplying a capacitive heater with a capacity of min. 240 dm3 in quantity of 206 pcs,

- Set II: 3 flat solar collectors, supplying a capacitive heater with a capacity of min. 280 dm3 in amount of 549 pcs,

- Set III: 4 flat solar collectors, supplying a capacitive heater with a capacity of min. 371 dm3 in amount of 60 pcs

8. List of statements or documents confirming the fulfillment of the conditions for participation in the procedure and no grounds for exclusion

To confirm the conditions of participation in the procedure and the lack of grounds for exclusion, the contractors shall submit:

8.1. together with the offer, JEDZ's statement in electronic form with a qualified electronic signature in order to pre-confirm the conditions of participation in the procedure and the lack of grounds for exclusion. Statements of entities submitting the offer jointly and entities sharing the potential submitted on the JEDZ form should be in the form of an electronic document, signed by a qualified electronic signature by each of them to the extent that they confirm the circumstances referred to in art. 22 paragraph 1 of the PPL Act, i.e. meeting the conditions for participation in the procedure and no grounds for exclusion to the extent that each of the contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion (Contractors may jointly demonstrate compliance with the conditions for participation in the procedure, subject to provisions : IDS SIWZ, first indent, IDW SIWZ, 5.2.1.3, IDW SIWZ, first indent, IDW SIWZ, first indent, 5.2W.3 IDW, SIWZ, first indent; no separate grounds for exclusion are listed separately).

8.1.1. The Purchaser allows in particular the following format of sent data: .pdf, .doc, .docx, .xps, .odt, rtf.

8.1.2. The contractor fills in EAT, creating an electronic document. Can use the ESPD tool or other available tools or software that allow you to fill in the EAT and create an electronic document, in particular in one of the abovementioned formats.

8.1.3. After the contractor has created or generated the EAT electronic document, the contractor signs the abovementioned a document with a qualified electronic signature issued by a qualified trust service provider that is an entity providing certification services - an electronic signature that meets the security requirements set out in the Act.

8.1.4. The contractor is obliged to complete the EAT form in the scope of: Part II, sections: A, B, C, D; Part III sections: A, B, C, D; Part IV section: α; Part VI - signature.

PART II - delivery and assembly together with commissioning of 295 installations with an air heat pump: implementation of 295 installations with an air heat pump for the needs of hot water or central heating in private buildings or on land belonging to the residents of partner municipalities consisting in the delivery of elements of the installation and carrying out on site assembly of air heat pump installations together with commissioning and user instructions. Five types of air heat pump installations are envisaged, depending on their minimum power (sets), in quantities:

- Set I: air heat pump working for the needs of heating domestic hot water (DHW) with a heat output of min. 1.7 kW, 174 pcs,

- Set II: air heat pump with a minimum heat output of 7.70 kW, in the quantity of 12 pieces;

- Set III: air heat pump with a minimum heat output of 13.10 kW, 72 pieces,

- Set IV: air heat pump with a minimum heat output of 19.70 kW, 29 pieces,

- Set V: air heat pump with a minimum heat output of 25.30 kW, 8 pieces.

8.2. Within 3 days of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, the Contractor shall provide the Ordering Party, without an additional request, with a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law in order to demonstrate the lack of grounds for exclusion from the public procurement procedure in the circumstances referred to in art. 24 paragraph 1 point 23) of the PZP Act, according to template No. 4 to IDW. Along with submitting a declaration of belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law, the contractor should provide, under pain of exclusion from the contract award procedure, evidence that links with another contractor do not lead to distortion of competition in the contract award procedure. The statement should be submitted in the original in the form of an electronic document bearing a qualified electronic signature or an electronic copy of the copy of the document confirmed to be true to the original.

8.3. Before awarding the contract, the contracting authority will call the contractor whose tender has been rated the highest in each of the lots to submit within the prescribed period, not shorter than 10 days, valid as at the date of submission of the following statements or documents:

8.3.1. To confirm compliance with the condition referred to in point 5.2.1.2., In point 5.2.2.2., In point 5.2.3.2., And in point 5.2.4.2, respectively. PART III - delivery and assembly with commissioning of 133 pellet boilers installations: execution of 133 installations of pellet boilers in private buildings belonging to the inhabitants of partner communes consisting in the delivery of installation elements and on-site assembly of pellet boilers installations together with commissioning and instruction. Four types of pellet boilers installation are anticipated, depending on their minimum power (sets), in quantities:

- Set I: pellet boiler with a minimum heat output of 12 kW, 40 pieces,

- Set II: pellet boiler with a minimum heat output of 16 kW, 29 pieces;

- Set III: pellet boiler with a minimum heat output of 24 kW, 58 pieces,

- Set IV: pellet boiler with a minimum heat output of 32 kW, 6 pieces.

8.3.2. In order to confirm compliance with the condition referred to in the first indents, respectively, in item 5.2.1.3., In item 5.2.2.3., In item 5.2.3.3., And in item 5.2.4.3. ToR - a list of deliveries or construction works carried out not earlier than in the last 5 years before the deadline for submission of tenders, and if the period of business activity is shorter - in this period, together with their value, subject, date of execution and entities for whom the deliveries or works have been carried out, accompanied by evidence stating whether these deliveries or works have been properly performed, the evidence in question being references or other documents issued by the entity for which the deliveries or works were carried out, and if for a justified reason of an objective nature, the contractor is unable to obtain these documents - other documents - Annex 3 to IDW (List of deliveries). Certificate of the competent head of the tax office confirming that the contractor is not in arrears with payment of taxes, issued not earlier than 3 months before the submission of bids, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines , in particular, he obtained the exemption, deferral or installment of overdue payments provided for by law or withholding in full the decision of the competent authority. 8.3.6.
