“The provision of transport services for the Joint Stock Company “Spa Krynica-Żegiestów” in Krynica — Zdrój ul. Nowotarskiego 9/4; 33-380 Krynica-Zdrój”

Package 1 — transport service 33-380 Krynica Zdrój ul Nowotarskiego 9/4 - buffer warehouse 32-742 Bochnia ul. Na Buczków 4

The Employer requires that the transport of goods and products be carried out under the following conditions:

FRACHT - transport 30-33 pallets between warehouses in Krynica-Zdrój (9/4 Nowotarskiego Street) and Bochnia (4 on Buczków Street)

Package 2 - transport of goods - whole vehicle freight in the area:

a) Lesser Poland Voivodeship - approx. 80 000 km — 1- 20 cargos/week - which is 35%

the value of the whole package

b) Greater Poland Voivodeship - approx. 15 000 km — 1-10 cargos/m-c - which is 7%

the value of the whole package

c) Masovian Voivodeship - approx. 25,000 km — 1- 8 cargos/week - which is 11%

the value of the whole package

d) Łódź Voivodeship — approx. 60 000 km — 1- 12 cargos/week - which is 25% of the value of the whole package,

e) Subcarpathian Voivodeship - approx. 2 500 km — 1-5 cargos/m-c - which is 1% of the value of the whole package,

f) Silesian Voivodeship - approx. 45 000 km — 1-8 cargos/m-c - which is 20% of the value of the whole package,

g) other voivodeships - approx. 2 500 km — 1-2 cargo/m-c - which is 1% of the value of the whole package.

The contractor reserves the opportunity to order the contractor to transport more than one unloading place.

1. The contractor is obliged to guarantee a minimum of 2 vans, including one with a lift with a capacity of up to 3.5 tons.

2. Area of operation max 120 km from UKŻ headquarters.

3. The Ordering Party can order 1-4 courses/week towards Krynica Zdrój-Rabka-Zakopane
