Greater East Midlands Commissioning Support Unit (GEM CSU) on behalf of NHS Nottingham North and East CCG, NHS Nottingham West CCG, NHS Rushcliffe CCG and NHS Nottingham City CCG is seeking suitably qualified providers to deliver Community Beds Services.
CCGs are looking for the provider to be the co-ordinator the provision of residential/nursing beds, and will look for the provider to be responsive in any subsequent reviews of community capacity and any additional beds required. CCGs perform an on-going review of demand and there is a possible increase in beds from October 2014.
Providers are required to deliver high quality and efficient services. Community Beds is a key component of integrated care, offering a short term step up and step down rehabilitation service. The priority target groups for this service are people who would 1. otherwise face unnecessarily prolonged hospital stays 2. have inappropriate admission to acute inpatient care, long term residential care 3. have continuing non-acute NHS in-patient care Subsequent guidance extended the remit to include: — adults of all ages, such as young disabled people managing their transition to adulthood; — homeless people and prisoners; and — older people with mental health needs, either as a primary or secondary diagnosis. — A renewed emphasis on those at risk of admission to residential care. — Timely access to specialist support as needed.
