1. The subject of the contract is the delivery to the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101, laboratory reagents and consumables according to the quantitative and generic list specified in Appendix No. 2 to the Specification of Terms and Conditions of the Contract, hereinafter referred to as the "Terms of Reference", together with the lease of analysers meeting the operational and utility parameters specified in Appendix No. 2 to the Terms of Reference, under the conditions specified in the draft provisions of the agreement constituting Appendix No. 4 to swz.

2. The laboratory reagents and consumables offered must be new and unused.

3. Delivery of the item of the order together with transport and unloading will be carried out at the expense and risk of the Contractor.

4. The Contractor is obliged to provide the Contracting Authority with a refrigerator for storing the offered reagents as part of the price of the subject of the contract for the duration of the contract.
