The subject of the contract is the successive delivery of medicines to the Drug Programs to the Dr. Karol Jonscher Municipal Medical Center in Łódź, divided into the following Packages:

Package I — Drugs for the Drug Program for the Treatment of Multiple Sclerosis,

Package II – Drugs for the Drug Program for the Treatment of Focal Dystonia and Hemiple Spasm of the Face;

Package III - Drugs for the Drug Program for the treatment of neovascular (exudative) form of age-related macular degeneration (AMD)

Package IV - Drugs for the Drug Program for the treatment of immunoglobulin transfusions in neurological diseases

The Ordering Party allows submission of partial offers in the above-mentioned Packages, i.e. with the possibility of submitting offers for individual items. A detailed description of the subject of the contract is set out in Annex 8 to the SWZ.
