The subject of the contract is the provision of nutrition services for patients of the St. John Paul II Mazovian Provincial Hospital in Siedlce Sp. z o. o. in the following locations: Siedlce, Poniatowskiego 26, 08-110 Siedlce and Rudka Aleja Teodora Dunina 1,

05-320 Mrozy and lease / lease of space in order to enable the Contractor to perform its obligations at the headquarters of the Contracting Entity. Detailed lease rules are set out in Annex 9 to the SWZ.

The execution of the contract will consist in:

preparation of meals according to the 14-day menus attached to the Agreement for the Procurement Agreement for diets indicated and described in point I of Annex 2 and 2a to the SWZ, taking into account the principles described below for individual diets, menus should contain seasonal options, an integral part of the menus will be recipes of dishes;

The execution of the contract will consist in:

1) preparing meals according to the 14-day menus attached to the Agreement for the Procurement Agreement for diets indicated and described in point I of Annex 2 and 2a to the SWZ, taking into account the principles described below for individual diets, menus should contain seasonal options, an integral part of the menus will be recipes of dishes; The Contractor shall be entitled to make modifications to menus and recipes during the term of performance of the contract only in justified situations, and the changes will require the approval of the Contracting Entity,

2) preparing meals in the expected amount on average 11 300 person-days per month in Siedlce and 6 200 person-days per month in Rudka and a maximum of 104 567 person-days in Siedlce and 57 870 person-days in Rudka throughout the term of the contract, in a manner consistent with the description set by the Ordering Party and provided in Annex 2 and 2a to the SWZ – Description of detailed requirements.

3) At the Siedlce location, the Contractor shall be obliged to:

a) providing meals to patients' beds and serving meals to individual patients in accordance with the diet determined by the doctor, in accordance with the indications contained in the further part of the document and as packed lunches for patients transported for examinations to places indicated by the Ordering Party in the demand;

b) serving meals to patients ensuring that the applicable temperatures, quality and hygiene are maintained,

c) collection of dishes, cutlery (along with food residues) after consumption,

d) washing dishes,

e) ensuring the collection and disposal of post-consumer residues and other waste generated in connection with the contract;

4) at the Rudka location, the Contractor will be obliged to:

a) provide meals ensuring that the applicable temperature, quality and hygiene are maintained, in GN's own catering containers, transported by its own means of transport, using its own thermoses suitable for the transport of hot and cold food; the service does not include the distribution of meals within the wards, i.e. serving meals to patients and collecting dishes from patients after meals, distribution of meals will be carried out by the Ordering Party using the bain-marie system,

b) ensure the collection of unwashed GN catering containers with thermoses by own transport,

c) utilization of post-consumer residues and other waste generated in connection with the contract.

5) The Contractor is obliged to perform the services in question with due diligence, in accordance with the provisions of this SWZ and, in particular, the applicable law, in particular the Contractor will be obliged to perform the contract in accordance with the provisions set out in the Act of 25 August 2006 on food and nutrition safety (i.e. Journal of Laws of 2023, item 1448) and executive regulations issued on the basis of this Act. The requirements resulting from the applicable law regarding the provision of services subject to this procedure are binding for Contractors, and failure to include them in this SWZ does not release the Contractors from the obligation to comply with them.
