Digital Patient Platform Framework Agreement

AZ Sint-Lucas wants to invest in technology that supports patient participation/empowerment.

We are looking for a partner to jointly develop a digital patient platform that accommodates all digital interactions between patient and healthcare providers/organization.

Our focus is on the following themes:

Self-management of the patient: As a hospital, we want to focus on increased self-management and, where desired, more control in the hands of the patient. With this we want to offer an answer to the increasing need of the patient to be actively involved on the one hand and on the other hand a growing scarcity in the care offer. To achieve this, a smart use of technology is crucial. For example, we want to make it possible, in preparation for a hospital contact, to have the patient answer a number of questions at home in peace, to have parameters measured at home or in the hospital, to watch an illuminating video or to make a list of the current medication. Furthermore, we want to give the patient the opportunity to pass on parameters on a continuous basis via a hospital app (and portal site) and associated devices, to keep a digital symptom diary, to consult, supplement or change their file.

Remote care: Remote care has made great strides due to the corona pandemic. The future platform should support our patients and caregivers to facilitate this care remotely. A clear tool/dashboard for our healthcare providers to guarantee this follow-up with accompanying alerts cannot be missing here. Specifically, this concerns continuous monitoring of vital parameters in the home setting, teleconsultations, questioning of medical outcome (PROMs), active sending out of digital education at certain moments in a care process. We expect to be able to create these care trajectories ourselves (WYSIWYG - interface), configure them and have them controlled (HL7, API) from our EHR. We also want to see the input (and/or a summary or processing of that input) of the patient reflected in our EHR.

Communication between patient/hospital organization : The digital patient platform becomes the forum where all communication between the patient, the caregivers and the hospital takes place. The patient will be able to indicate his preferences of communication here, both in terms of content (medical information / administrative / news / ...) and formally (SMS, hospital app, portal, e-mail, ...).

An important aspect of this assignment is the linking with the EPD. Demonstrable experience with links with an EHR and demonstrable experience with links with medical devices are an important element in the evaluation of the offered solution. In addition, when assessing the solution offered, the uniformity of the experience for the patient is paramount.
