1. The subject of the contract are supplies of narcotic and psychotropic drugs, specified in parts for the Fryderyk Chopin University Clinical Hospital in Rzeszów.

2. A detailed description of the subject of the contract, the requirements of the Ordering Party, information on the admission of other quantities of pieces in the package, are specified in the Price and Offer Forms – Appendices No. 1 for individual parts, which are an integral part of the SWZ.

3.The Ordering Party requires offer items of the order that meet the requirements described in SWZ.

4. The scope of the contract includes the delivery of the objects of the order in the specified quantities

in Price and offer forms for the period:

- Part 1-3 – 24 months

5.The Contracting Party shall not allow the submission of equivalent tenders.

Due to the program (duration of the contract) is given: 24 months, it should be:

- Part No. 1 - for 24 months from the date of conclusion of the contract.

In Part No. 1, the Contracting Authority allows for the possibility of changing the term of the contract concluded with the Contractor in the event of failure to complete the entire subject of the contract during its term. In this case, the amendment to the contract requires signing an annex, but the Contracting Authority unilaterally notifies the Contractor about the proposed extension of the contract before the expiry of the term for which it was concluded.
