County Durham and Darlington NHS Foundation Trust are looking for a service to be provided for the Provision and Delivery of Third Party Internet Provider Chlamydia Postal Packs to the Residents of County Durham aged 16-24. (At this time residents in Darlington with a Darlington Borough postcode are not commissioned to receive this service). The service must include the ability for the service user to request a postal pack via the Internet. The service user will then return their sample to the provider for the testing of chlamydia, the provider will then notify the service user and the Sexual Health Team at CDDFT of their test results.

The contract period is 12 months commencing 1.10.2016 (with the option to extend for a further 2 x 12 months)
