- task of No. 1: extension of the provincial road No. 755 from DK 74 to ul. Plażowa in Zawichost - stage I: Bałtówka-Kolęcin section at 23 + 084 km to 34 + 725 km,
- task of No. 2: extension of the provincial road No. 755 from DK 74 to ul. Plażowa in Zawichost - stage I: section DW 777 Zawichost - province border in km 15 + 994 to 18 + 405 (joint route with DW 755).
Scope of the service:
- management and coordination of investment implementation (design stage and construction works),

General characteristics of the investment
The investment task being the subject of supervision is implemented in the "design and build" system. The planned investment is located in the Świętokrzyskie voivodship in the Sandomierz poviat, Zawichost commune. The scope of the investment task being the subject of supervision includes:
a) Preparation of project documentation under the Functional Program (PFU) together with the necessary administrative decisions, opinions and arrangements permitting construction works for the investment task, including the environmental decision and the water permit for the construction of water facilities, demolition and construction of a new bridge facility. As part of the contract (at the contract price and time for completion), if necessary, it is also necessary to: - prepare a report and carry out an environmental impact assessment, - if necessary, derogate from technical conditions. To be used in the preparation of technical documentation, the Employer submits the design documentation "Reconstruction of DW 755 together with the construction of a pavement on the section from km 25 + 700 to km 27 + 400 (left side) in the city of Suchodółka". The documentation provided is ancillary.
b) Execution of construction works together with the necessary accompanying infrastructure based on the design documentation made by the Contractor of works together with services which are not construction works and provision of environmental supervision in the scope resulting from special provisions and decisions on the environmental conditions of the project, if necessary securing and relocation of monuments street furniture, ensuring archaeological supervision and conducting rescue archaeological research, pre-archaeological research (pre-excavation research), provision of sapper and geological supervision. The detailed scope of works is presented in PFU.
c) Exercising author's supervision over the developed project documentation.
d) Obtaining permission to use. As part of the investment it is necessary to design and perform:
- extension of DW 755 along the existing track with the possible correction of non-normative arches and the construction of pedestrian routes, bus bays, culverts, road drainage, construction / reconstruction of road lighting in accordance with applicable regulations, reconstruction of colliding devices and the network of existing infrastructure under and above ground, cutting of colliding trees,
- extension of DW 777 along the existing track with the possible correction of non-normative arches and the construction of a pedestrian route with asphalt surface, bus bays, reconstruction of intersections, road culverts and drainage, demolition and construction of a new bridge structure, construction / reconstruction of road lighting in accordance with applicable regulations, reconstruction of colliding devices and existing and underground infrastructure networks, felling colliding trees.
