1) The subject of the contract constitutes the supply of erythropoiesis stimulating agents for Dr. Ludwik Błażek Multispecialist Hospital in Inowrocław, within 12 months from the date of the signing of the contract.
2) The duration of the order specified for 12 months may be extended by the Contracting Authority in the event that the Ordering Party has not used the quantity of the assortment specified by the subject of the contract.
3) Delivery date of the current order — up to 72 hours maximum.
4) Place of execution of the order: Dr. Ludwik Błażek Multipecjalist Hospital, 97 Poznańska Street, 88-100 Inowrocław.
