the purpose of this contract is the implementation of a communicating file in urbanized regional oncology (Dcc) with the various cancer actors
the DCC is a digital file allowing, in particular, health professionals to have access to the information present in the key documents produced at each stage of the cancer care pathway: Anatomo-topathology reports (Cr-Acp), operative reports (Cro), which sign the diagnosis; The sheets of the Multidisciplinary Consultation Meetings (frcp) which establish the proposals for therapeutic care; The Personalized Care Program (Pps), defined as an immediate relay of the announcement device, which formalizes the adaptation of the therapeutic strategy decided in CPR to the patient's choices and preferences. The Personalized Follow-up and Surveillance Program, known as the Post-Cancer Program (Ppac). In particular, it formalizes the information to be brought to the attention of the attending physician at the end of the acute phase of treatment, and the specific care in conjunction with the city in order to facilitate the return to working life. The monitoring of the increase in DCC is done with the national authorities and aims to measure: The dematerialization of documents (all CPR and Pps sheets), a prerequisite for their rapid and complete dissemination to all those involved in patient care; Interoperability (the level of information structuring) of these documents
