The subject of the order consists of 7 packages. The contracting authority allows for the possibility of submitting partial offers for individual full packages. Each package is a separate partial offer. Each of the Contractors may submit a bid for any number of packages.
Package No. 1The subject of the contract is the purchase and delivery of reagents for the VITEK 2 COMPACT 60 apparatus

The Contractor shall fulfil the order under the terms and conditions described in the proposed contract provisions of Annexes 2 or 2A to the SWZ.
The Contractor is obliged to carry out the subject of the contract successively for 6 months from the date of conclusion of the contract.
Package No. 2The subject of the contract is the purchase and supply of reagents for the BACT ALERT 3 D 120 apparatus

The Contractor is obliged to carry out the subject of the contract successively 6 months from the date of conclusion of the contract.
Package No. 3The subject of the contract is the purchase and supply of reagents for serological tests II

The Contractor shall fulfil the order under the terms and conditions described in the proposed contract provisions of Annexes 2 or 2A to the SWZ.
The Contractor is obliged to perform the subject of the contract successively 8 months from the date of conclusion of the contract.
Package No. 4The subject of the contract are other reagents.

The Contractor is obliged to perform the contract on the terms and conditions described in the draft provisions of the contract constituting Annexes 2 or 2A to the SWZ.
The Contractor shall be obliged to carry out the subject of the contract successively 12 months from the date of conclusion of the contract.
Package number 5The subject of the contract is the purchase and delivery of tests: Enzyme-specific immunosorbent assays for the determination of IgE specific in the panel system, AUTOIMMUNE PANEL for determining the presence of antibodies in IgA and IgG classes, supply and lease of equipment for performing tests
