1.The subject of the contract is: successive delivery of orthopedic implants to SP ZOZ Siemiatycze, in quantities and assortment specified in the assortment and quantity and price form, application to successive reported needs (orders) of the Ordering Party, packages from 1 to 6 SWZ, described in detail in Appendix No. 2, constituting an integral part of it, and then an integral part of the supply agreement constituting Annex 1 to the contract.
