Shropshire County PCT invites formal expressions of interest for the provision of a Community Bed Based Intermediate Care Service (Step up care and rehabilitation) in Shrewsbury & Atcham.

This service is an essential part of the unscheduled care and reablement pathways for people living in the Shrewsbury & Atcham locality. The Provider will be responsible for the provision of beds, nursing, medical (GP) support and rehabilitation including physiotherapy and occupational therapy. The service will be for a maximum of 15 patients at any one time. This will include the following:

— 5 step up beds,

— 10 rehabilitation/reablement beds.

The service will reflect the principle of the delivery of person centred care. The provider will have experience of delivering care to people with a variety of complex needs with a focus on enabling the person to maintain/regain their optimal level of independence.
