Delivery, implementation, productive commissioning and operation of software for digital discharge and transfer management in accordance with the Hospital Future Act Funding Grant 2 (KHZG FTB 2)
The tender includes the delivery, implementation, productive commissioning and operation of software for digital discharge and transfer management in the UKA in accordance with the requirements and mandatory criteria of the KHZG FTB 2 (§19 para. 1 sentence 2 KHSFV). The implementation includes a connection to the existing hospital information system (HIS) as well as an integration into the IT landscape of the UKA. The digital discharge and transfer management enables the UKA to find a suitable follow-up care provider for patients on time. In addition, it serves to pass on information about treatment data to aftercare providers, such as rehabilitation and care facilities as well as suppliers of medical aids. The use of digital technologies reduces the administrative communication effort and accelerates the transition process. The following specific functions, among others, are to be made possible: Structured data exchange between service providers and aftercaregivers, reporting and querying free capacities of aftercaregivers and storage of patient data in the electronic patient file.
