Description:
Delivery of transplant materials

The subject of the contract is the supply of materials for transplantation to the L. Zamenhof University Children's Clinical Hospital in Białystok, 17 J. Waszyngtona Street, hereinafter referred to as the Ordering Party. A detailed description of the order is described in the Offer Form - Annex 1 to the SWZ

Package 1

1. Anatomical plate for distal epiphysis of the humerus, elbow and ulnar process variable angle a "plate for the ulnar process . Anatomical reconstruction plate with a shape that reduces contact with the bone, locking and compression with a variable-angle lock. In the head of the plate there are thickened holes made of four threaded columns with the possibility of using variable-angle locking screws with a deviation from the axis in each direction up to 15 degrees, with a diameter of 2.7 mm, with a threaded head or alternatively standard cortical screws with a diameter of 2.4 mm. Locking screws with a tapered thread on the head screwed in with a 0.8/1.2NM torque screwdriver. On the shaft of the slab from the bottom and side there are undercuts for easy remodeling of the slab. On the shaft there are also dual-function, locking and compression holes with the possibility of using a single 3.5mm locking screw or a cortical/spongy screw with a diameter of 3.5/4.0mm. The appropriate shape of the holes in the plate gives the possibility of compression between the splinter and the longitudinal locking and compression hole allows vertical positioning of the plate. The shape of the holes on the shaft of the plate also allows the use of the pull screw technique. The core diameter for 3.5mm locking screws is 2.9mm; cortical fin of 3.5mm is equal to 2.4mm. Instruments equipped with: drill bits with AO tip; screwdriver inserts with an AO tip for a AO type quick drill fitting or a 0.8/1.5NM dynamometer; 0.8/1.5NM dynamometer with the possibility of attaching to a quick AO drill fitting or an external AO bit holder. The plate system works with 3.5mm perforated screws for augmentation. Steel implants made of material approved for magnetic resonance imaging. Appendix plates are available in three versions: small, medium and large ulnar ends. Right and left plates:

Package 2

1" Flexible intramedullary nails for molar and epiphyseal fractures. A full-length straight nail with a flattened end bent at different angles depending on the diameter of the nail, making it easier to insert. Made of titanium and color coded according to diameter.

Diameter 1.5 - 4.0mm with the possibility of cutting to the desired length."

1) The Contractor shall lend free of charge for the duration of the contract instruments for implantation and removal of flexible nails and screws for the tarsal sinus.

2) The Contractor shall create a "bank" of nails and screws for the tarsal bay at the Contracting Authority's premises within the range of assortment and dimensions agreed between the parties.

Package 3

1 Reinforced surgical suture size #0, needle 22.2mm, bulk pack of 12 pieces 1

2 Titanium plate, sterile, dedicated to AC joint reconstruction, rectangular in shape with rounded corners. On both sides, there is an indentation with a hole for loading 2 mm wide non-absorbable specialist tapes. The implant is anatomically bent to the surface of the clavicle and coracoid process with a laser line marking the axial alignment of the implant in relation to the bone. Pcs. 10

3 One-time knotless set for AC Joint repair. The system consists of a rectangular-shaped button mounted on a disposable feeder loaded with a strong #6 thread to lock the anastomosis. Pcs. 1

4 Anterior ACL and posterior PCL ligament reconstruction system based on screws. A biocomposite screw for fixing the graft in the canal, made of 30% calcium diphosphate and 70% PLDLA. Screw with soft thread along the entire length. Socket conical screws (for easier and more precise insertion). Sizes: Length 23 mm with 6 diameters, packed with cover sleeve. Pcs. 50

5 Disposable needle for reusable knee scorpion sewing tool. The needle is used to feed the thread to the upper jaw of the tool. Needle packed sterile pcs. 8

6 Nodal implant made of tube-shaped thread with a diameter of 2.6mm. An implant placed on a disposable feeder constructed in a self-punch system that allows the anchor to be implanted without prior drilling or tamping of the bone to form a lodge. The anchor is loaded with one black and white tape with a 1.7 mm wide loop and one 1.3 mm wide white and blue tape. The belts are made of polyester braided with UHMWPE - ultra-high molecular weight polyethylene. Pcs. 18

* The Contractor will lend free of charge for the duration of the contract the instruments necessary to carry out the procedure.

Package 4

Arthroscopy Implant Kit

1 "

• 20mm elongated endobutton which is an overlay on the endobutton used for revision procedures

• Round endobutton in sizes 15, 17, 21 mm

• Oval endobutton in sizes 9x16 mm, 11x18 mm, 15x21 • plate with 8 holes made of titanium alloy in the shape of a rectangle with rounded sides 12 mm long, permanently connected with a thick loop protecting the graft, made of non-absorbable UHMWPE suture, allowing the graft to be suspended in the femoral canal and with threads for pulling the implant to the external cortex. The loop for pulling up the graft must be able to reduce the length of the loop in the range of 90 mm - 10 mm with one hand. The implant is pre-loaded on a cardboard box, making it easier to place the graft." 10

2 Endobutton in the form of a button, for tibial fixation, with an ultra-low profile, in a shape similar to a circle, in three sizes (12, 15, 18 mm) with adjustable loop, for tibial canals with diameters from 4.9 to 13 mm, designed to protect the graft while reducing the loop, by minimizing the relative movement of the graft and avoiding the "sawing" effect. Ability to reduce loops with one hand. Pre-assembled, integrated with #2 thread and needle - included. For "all inside" ACL treatments. Also endobutton versions for quadriceps tendon fixation, BTB and QT-B. Pcs. 3

8 Sterile, disposable, ready-to-use soft tissue suturing tool with a pre-loaded needle inside that curves upwards during application and passage through the tissues, pulling the thread or tape behind it, and the upper jaw intercepts it. Pistol grip with trigger. Diameter of the working shaft 5.2 mm, width of the working tip 5.2 mm, length of the working tip 18.4 mm, thickness of the working tip 5.4 mm.
