Description:

NHS Brent Clinical Commissioning Group is inviting responses to provide a pathway of care for patients transferred from an acute inpatient environment, or where a patient requires a greater level of care than can be provided in their home, to an intermediate care inpatient setting to continue a period of rehabilitation or recuperation with an aim of returning to their usual place of residence.

These thirty five (35) beds are specifically for patients requiring a short term Programme of intensive rehabilitation therapy/care for up to a maximum of 28 days in a community hospital or residential setting for people who are medically stable (no significant medical problems that require acute medical care) but need a short period of rehabilitation from skilled nurses and therapists to enable them to re-gain sufficient physical functioning and confidence to return safely into their own home/place of usual residence with or without further therapy input delivered in an OP setting.



The Contracting Authority is seeking to provide sub-acute clinical care and rehabilitation for patients with a range of general conditions in order to assist them to achieve their:

— maximum potential for physical, cognitive, social and psychological function, enhance their participation in society and improve their quality of life;

— to provide an alternative to treatment in the home or acute hospital for those patients requiring sub-acute clinical and/or rehabilitation support which can only be provided in an intermediate care bed in order to prevent admission to an acute facility or reduce the acute length of stay for patients who have been admitted and no longer require acute care:

—— to enable timely patient discharge from acute hospital at the earliest opportunity;

—— to enable early transfer for rehabilitation intervention to prevent and manage avoidable complications;

—— to maximise patient independence, function and mobility;

—— to facilitate safe and timely patient discharge to their home or anticipated discharge destination;

—— to provide a time-limited service where there is no practical or appropriate alternative to a community rehabilitation bed;

—— to provide a responsive service with a strong clinical skillset to support patients with exacerbations/in need of crisis intervention whose needs can be met in a bedded intermediate care setting;

—— to reduce the hand-offs between the different services to ensure the smoothest and safest flow for patients

—— to actively promote independent living, development of self-care skills and resilience of service users and that of their informal carers and families in the community.

The contract to be offered to the successful bidder at the end of this procurement will be for a duration of three (3) years, with the possibility of one extension, up to a further two (2) years, subject to any restrictions placed nationally by NHS England or the Department of Health.

The services to which this procurement relates fall within Schedule 3 of the Public Contracts Regulations 2015 (the “Regulations"). As such, this procurement is being run as a bespoke, single-stage application process akin to the open procedure.
