Services ensuring the maintenance of medical devices constituting the equipment of the Multi-Specialist Hospital in Inowrocław in accordance with the provisions of the Medical Devices Act of 20 May 2010. (OJ 2020, item 186)

1) The subject of the order includes the performance of repairs, maintenance, service activities, software updates, reviews, regulations, calibration, calibration, checks and safety checks of medical devices, the list of which is annex 2 of the SWZ according to:

- requirements of the applicable provisions, including the Act of 20 May 2010 on medical devices (OJ 2020, item 186),

- manufacturer's guidelines, technical and motion documentation (equipment, apparatus), attached to the product by the manufacturer with instructions, technical conditions, applicable standards and in compliance with health and safety regulations, as well as the procedures in force with the Contracting Authority resulting from the applicable Quality Management System.

1) The subject of the order includes the performance of repairs, maintenance, service activities, software updates, reviews, regulations, calibration, calibration, checks and safety checks of medical devices, the list of which is annex 2 of the SWZ according to:

- requirements of the applicable provisions, including the Act of 20 May 2010 on medical devices (OJ 2020, item 186),

- manufacturer's guidelines, technical and motion documentation (equipment, apparatus), attached to the product by the manufacturer with instructions, technical conditions, applicable standards and in compliance with health and safety regulations, as well as the procedures in force with the Contracting Authority resulting from the applicable Quality Management System.

2) In addition to the subject of the contract described in point 1, it includes the issuance of judgments on the technical condition of medical devices on order of the Contracting Authority.

3) The services covered by the contract, except for repairs and technical condition judgments, shall be carried out on the basis of a timetable developed by the Contractor and annexed to the tender under the terms specified in the SWZ. The schedule should include individual actions and deadlines for their execution with a date.

4) Repair services without spare parts a maximum of 3 days, repair time with replacement of spare parts up to 5 days and response time of not more than 48 hours from the date of notification by the Contracting Authority, the notification may be made by telephone or e-mail around the clock — 365 days a year.

5) The services covered by the contract will be performed at the premises of the Contracting Authority, using control and measurement apparatus, tools and materials of the Contractor, with the consent of the Contracting Authority authorising the possibility of performing the service outside the Contracting Authority's premises.

6) The performance of the services will be carried out on the basis of a namesake authorization issued by the Contractor, who has the authority to perform the service and report to the Medical Devices Repair and Maintenance Department of the Ordering Party — on working days during working hours and outside those hours and the place of performance of the service must be agreed with the direct user in accordance with the order of the service.

7. Whenever the concept of “review” occurs in the content of the SWZ and the template of the contract, it is appropriate to mean the activities covered by the contract referred to in Part II (1) of the SWZ, except for repairs.
