The subject of the contract is the paid provision by the Contractor of a group life insurance service for employees of the District Prosecutor's Office in Gliwice and its subordinate units and their family members.

The subject of the contract are voluntary life insurance services for employees of the District Prosecutor's Office in Gliwice and its subordinate units and their family members. A detailed description of the subject of the contract is included in Annex 1 to the ToR - description of the subject of the contract. In the annex the Employer specified the required insurance conditions. The scope and conditions of insurance protection described in the ToR and annexes are the scope and mandatory conditions, minimal acceptable to the Contractor. The obligatory scope of insurance protection, determined by the type and amount of benefits for four groups of benefits: option I, option II, option III and option IV is included in chapter VI of Annex 1 to the ToR.
