The subject of the contract are successive deliveries of reagents, consumables, laboratory equipment, tests and lease of medical devices to the laboratory according to the needs of the Ordering Party, divided into 10 parts, hereinafter referred to as packages. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

The Contracting Authority allows partial bids for any number of packages. However, the Contracting Authority reserves the right to submit offers by the Contractor for the entire assortment in a given Package. Each part of the order will be considered separately.

Package 1. Disposable laboratory equipment. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 10. Lease of an immunoassay analyser with the supply of reagents and consumables for the leased apparatus and lease of a replacement apparatus, installation and lease of a laboratory centrifuge with an angular rotor for at least 36 tubes of not less than 5 ml and the possibility of setting the spin time by second. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 2. Latex reagents, rapid immunochemical tests and dyes. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 3. Lease of a critical parameters analyzer with the supply of reagents and with the lease of a replacement apparatus for gasometry and the possibility of attaching electrolyte determinations. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 4. Syringes for gasometry. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 5.Lease of a multifunctional automatic biochemical analyzer with replacement apparatus and centrifuge. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 6. Reagents for the determination of blood groups. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 7. Closed blood collection system. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 8. Lease of an analyzer for Real time PCR panel tests along with the delivery of tests. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.

Package 9. Delivery of substrates for the BactAlert 3D60 camera. A detailed description is attached as Appendix 2 to the SWZ – Assortment and price forms.
