4) instead of draquets — film-coated tablets or tablets or capsules,
5) instead of tablets, film-coated tablets or slow release capsules — film-coated tablets or modified release capsules and vice versa.

The subject of this part of the contract is the choice of the contractor who will provide supplies to the contractor consisting in the successive delivery by the contractor to the Dr. Tytus Chałubiński County Hospital in Zakopane, Kamieniec Street 10, 34-500 Zakopane — Pharmacy Szpitalna, pharmaceuticals (medicines) in the scope of Package No 7 — 2 assortment items according to the generic and quantitative statement set out in point 4.10 of the present EGM.
