1. The subject of the contract is "Expansion of the IT system" as part of the project entitled "Małopolska Medical Information System (MSIM)" covering the delivery and implementation of software: 1) Mobile application of the medical system - open 2) User desktops - open 3) Treatment rooms - open 4) System of supplying the hospital database with a digital version of medical documentation created in paper form – multi-station – 1 pc.

3. The subject of the contract includes the delivery and implementation of software, under which the Contractor: 1) will deliver and launch (delivery, installation, configuration, author's supervision) software: a) Mobile application of the medical system, b) User desktops, c) Treatment rooms, 2) in the IT infrastructure of the Ordering Party in a virtualized platform, 3) provide, install and configure software intended for digitization of archival and current documentation medical system created in paper form related to the process of treating patients and saving its digital version in the EDM Repository: a) The hospital database supply system with a digital version of medical documentation created in paper form – multi-user 4) grants an indefinite license for the software: Mobile application of the medical system, User dashboards, Treatment rooms, Hospital database power supply system with a digital version of medical documentation created in the form of paper – multi-station, 5) integrate the delivered software listed in point 1) with domain systems owned by the Ordering Party: HIS System – AMMS by ASSECO Poland, EDM Repository - AMMS by ASSECO Poland, with the LIS System in the area of the Analytical Laboratory - Infomedica by ASSECO Poland, LIS System in the area of Microbiology Laboratory - Infomedica by ASSECO Poland, RIS/PACS Infinitt system, 6) will integrate the delivered software listed in point 2) with domain systems owned by the Ordering Party: HIS – AMMS system by ASSECO Poland, EDM – AMMS repository by ASSECO Poland. The implementation will be carried out on the infrastructure of the Contracting Entity, which will be made available to the Contractor in order to carry out the activities subject to these proceedings. The Ordering Party shall provide a virtualized server under the control of the VMware system, prepared according to the Contractor's instructions (RAM, processor, disk space).
