The subject of the contract are motor vehicle insurance with equipment in the scope of:

1.1. compulsory civil liability insurance of motor vehicle owners referred to in the Act of 22 May 2003 on compulsory insurance, the Insurance Guarantee Fund and the Polish Motor Insurers' Bureau (consolidated text, Journal of Laws of 2022, item 2277, as amended) ("OC");

1.2. Auto Casco insurance ("AC");

1.3. accident insurance for drivers and passengers ("NNW");

1.4. Assistance Insurance ("ASS");

1.5. insurance of civil liability of owners of motor vehicles in foreign traffic ("ZK");

1. The subject of the Basic Order are insurance services for the Units, within a period of 36 months calculated separately for each motor vehicle submitted in the first settlement period under the public procurement agreement, in the following scope:

1.1. compulsory civil liability insurance of motor vehicle owners, referred to in the Act of 22 May 2003 on compulsory insurance, the Insurance Guarantee Fund and the Polish Motor Insurers' Bureau (consolidated text, Journal of Laws of 2022, item 2277, as amended) and in Section II Chapter 1 of Annex 1 to the Agreement "Description of the subject of insurance", hereinafter also referred to as 'third party liability insurance' or 'third party liability insurance';

1.2. Auto Casco insurance referred to in Section II Chapter 2 of Annex 1 to the Agreement "Description of the Subject of Insurance", hereinafter also referred to as "AC insurance" or "AC";

1.3. insurance of accidents of drivers and passengers, referred to in Section II Chapter 3 of Annex 1 to the Agreement "Description of the Subject of Insurance", hereinafter also referred to as "accident insurance" or "accident insurance";

1.4. Assistance insurance referred to in Section II Chapter 4 of Annex 1 to the Agreement "Description of the Subject of Insurance", hereinafter also referred to as "ASS insurance" or "ASS";

1.5. civil liability insurance of owners of motor vehicles in foreign traffic, referred to in Section II Chapter 5 of Appendix No. 1 to the Agreement "Description of the Subject of Insurance", hereinafter also referred to as "ZK insurance" or "ZK".

2. Periods of motor vehicle insurance are indicated in the "List of vehicles" constituting Annex 9 to the SWZ "List of vehicles - calculation of costs".

3. A detailed description of the subject of the Order is specified in the model Agreement "General Agreement of motor insurance" constituting Annex 5 to the SWZ and

in Appendix 1 to the model Agreement "Description of the Subject of the Contract" constituting Annex 2 to the SWZ;

4. The Contracting Authority, acting pursuant to Article 95(1) of the PPL, requires the Contractor or Subcontractor to employ on the basis of an employment relationship all persons who will be responsible for servicing the Agreement and will perform activities consisting in: ongoing contact with the Policyholder, the Insured or a broker (correspondence under the Agreement and telephone contacts in matters related to the Agreement), issuing insurance documents, settlement of the Agreement, confirmation of the scope of the Insurer's liability, confirmation of insurance protection, informing about the status of insurance premiums settlements, providing information or explanations related to the implementation of the Agreement, on the terms and to the extent specified in the model Agreement constituting Annex 5 to the SWZ.

5. The meaning of the terms used in this SWZ is identical to the definitions adopted in the model Agreement constituting Annex 5 to the SWZ and the Description of the subject of the contract constituting Annex 2 to the SWZ.

6. The list of Entities for which proceedings are conducted is included in Appendix 8 to the SWZ.

7. Detailed data on Units and vehicles necessary to assess insurance risk, including data on the subject of insurance and claims, are included in Annexes to the SWZ No. 8 to No. 10.
